Who I am

This website is operated by Ibrahim Afyon, a New York–licensed insurance agent and broker (licenses LA-1934647, PC-1934647, BR-1934647; NPN 21711447) and Registered Social Security Analyst (RSSA® #9932), located at Clifton Park, NY 12065. What I collect

Information you give me. If you fill in the contact form or call, I collect what you choose to share: typically your name, phone number, ZIP code, an email address if you add one, which topic you want to discuss, whether you ticked the box allowing text messages, and the version of the consent wording you saw. If you send me the results of one of the coverage checks on this site, that summary is attached to your request. The Medicare plan finder works the same way: pressing “Send my answers” adds the answers you chose to the contact form, and they reach me through Netlify Forms only when you submit that form. They are your county, the hospital you want to be able to use, whether your doctors are in one health system, whether you spend part of the year away, what your prescriptions look like, whether you have Medicaid, a Medicare Savings Program or Extra Help, how you feel about referrals and prior authorization, and whether you have retiree, union or VA coverage, together with the results the finder showed you. If we go on to work together, I may collect the details needed to compare or enroll you in a plan, such as your date of birth, Medicare number, doctors and prescriptions, or — for business insurance — details about your business, payroll and vehicles. I never ask for Medicare numbers or health details through this website, and you should not send them, or business, payroll or vehicle details, through the contact form; for property and casualty quotes I use the personal questionnaire link described below.

Quote questionnaire. If I'm quoting car, home, renters, condo, landlord, umbrella or business insurance for you, I may send you a personal link to a questionnaire at quote.afyoninsurance.com. It collects the answers you choose to give, which can include your name and contact details; your vehicles and their VINs; drivers' names, dates of birth and license details; property details; your current insurance and claims; and, for a business, details such as its federal tax ID (FEIN), payroll, employees, vehicles and equipment. It also collects the documents you choose to upload, such as declarations pages, insurance ID cards, driver's licenses, loss runs, payroll summaries and contracts. The questionnaire never asks for your Social Security number, bank or card numbers, or medical history, and it removes anything that looks like a Social Security or card number from what you type. It has no analytics or advertising cookies, and it is not used for Medicare. More detail is at quote.afyoninsurance.com/privacy.html.

Information collected automatically. Like most websites, my hosting provider keeps standard server logs (IP address, browser type, pages visited, time of visit). This site also uses Google Analytics 4 to count visits, see which pages are read, and see how many people tap the phone number, WhatsApp, email or the contact form. It is configured with Google Signals and advertising personalization switched off, so it is not used to build a profile of you across other websites, and IP addresses are not stored by Google Analytics. You can opt out with Google's browser add-on at tools.google.com/dlpage/gaoptout. When the contact form goes through, the analytics service also receives an event with the topic you picked from the list, whether a check summary was attached (yes or no) and the page address; a reminder sign-up sends only the page address, and finishing or sending one of the checks records only which check it was. Your name, phone number, email address, answers and anything you type are never sent to analytics.

This website does not use advertising cookies and does not sell your information; the only cookies are the analytics ones described above. The coverage and claim checks and the calculators keep your answers in your own browser's storage so a refresh doesn't lose them; that data never leaves your device unless you choose to send it.

How I use it

• To respond to your enquiry and set up a conversation.
• To compare Medicare, Social Security or other insurance options with you, if you ask me to.
• To submit an application or enrollment you have chosen to make, and to remain your point of contact for it.
• To keep the records that New York State and the Centers for Medicare & Medicaid Services require licensed agents to keep.

I do not use your information for purposes unrelated to your enquiry, and I do not add you to a mailing list unless you ask to be added. If you sign up for the enrollment-window reminder, I keep only your email address, the page you signed up on and that page's language (English or Turkish), and use them to send one note before each Medicare enrollment window; reply "stop" to any of them and I delete the address.

Who I share it with

Only where it is needed to do what you asked. That means the insurance carrier whose plan or policy you choose; the Field Marketing Organization that supports my Medicare carrier appointments, where they process the enrollment — and where Medicare rules require your express written consent before your details go to another marketing organization, I obtain it through a separate disclosure naming the recipient; and service providers who run this website, the contact form, or my email (the site and its contact form are hosted by Netlify, Inc., which stores form submissions and forwards them to my inbox; if you book a call through the scheduling page, the name, phone number and note you enter are processed by Calendly, LLC under its own privacy notice and forwarded to my calendar). If you use the quote questionnaire, your answers are saved to my client database at Airtable, Inc., and the documents you upload are stored in private storage at Netlify, Inc.; smaller documents are also attached to your record in Airtable. Both act only as my service providers. Netlify screens form submissions for spam with Akismet, a service of Automattic Inc., which receives the submission for that check. If you message me on WhatsApp, your messages are carried and stored by WhatsApp, a Meta Platforms service, under WhatsApp's own privacy policy. I do not sell, rent or trade your information, and mobile phone numbers and text-message consent are never shared with third parties or affiliates for their marketing. I may disclose information if required by law or by a regulator such as the New York State Department of Financial Services.

Medicare-specific rules

Medicare marketing is regulated by the Centers for Medicare & Medicaid Services. Before I discuss specific Medicare Advantage or Part D plans with you, I will ask you to complete a Scope of Appointment form so that our conversation stays within the products you want to discuss. I will not contact you about Medicare plans unless you have asked me to, and you can withdraw that permission at any time. Calls in which Medicare Advantage or Part D plans are discussed are recorded, as CMS requires of agents, and the recordings are kept for the period CMS requires and shared only with the carrier or CMS if they ask for them.

How long I keep it

Ordinary enquiries that do not lead to an engagement are generally deleted within 12 months, except that the record of your consent to be contacted (the form submission itself) is kept for as long as the rules on telephone and text consent may require. Records subject to legal or contractual retention rules — including Medicare marketing and sales call records, which CMS requires agents to keep whether or not you enroll — are kept for the required period, currently ten years for Medicare enrollment records. Medicare marketing and sales calls, including the audio portion of calls made over the internet, are recorded and kept for at least 6 years, as Medicare requires (42 CFR 422.2274(g)(2)(ii)). Answers you give in the quote questionnaire become part of your client record and are kept on the same terms. Documents you upload there are deleted from the questionnaire's storage once I no longer need them to quote or service your policy. Personal questionnaire links expire, and they can't be reopened once you submit.

Your choices

• You can ask me what information I hold about you, ask me to correct it, or ask me to delete it where I am not required to keep it.
• You can ask me to stop contacting you at any time — by phone, or by replying to any message.
• You are never required to use the contact form; calling works just as well.

Security

This website is served over HTTPS. I keep client records in password-protected systems and do not store Medicare numbers or health information on this website. No method of transmission is perfectly secure, which is why I ask you not to send sensitive numbers or documents through the contact form, email, text or WhatsApp; when documents are needed, I send you a personal link to my quote questionnaire, which is served over HTTPS and stores uploads privately, or we agree on another secure way to share them.
