What's the best Medicare plan in 2027?
It's a fair question, and the honest answer is a set of questions back. Here is how to tell which plan is best for you, what the choice looks like in each county this year, and what is already known about next year's costs.
There is no single best Medicare plan for 2027, and anyone who names one without asking about you is guessing. The best plan for you is the one that covers your doctors and your hospital, covers your prescriptions at the lowest total cost for the year, and fits whether you would rather pay more each month or more when you use care. In the Capital Region that means choosing among 22 to 57 Medicare Advantage plans per county for 2027 (CMS landscape file, read October 27, 2026), of which 11 to 31 are open to anyone with Parts A and B and the rest are Special Needs Plans, or choosing Original Medicare with a Medigap plan and a Part D drug plan. Compare during the Annual Enrollment Period, October 15 to December 7, 2026, for coverage that starts January 1, 2027.
Why no one can honestly name one best plan
Three things decide which plan is best, and all three are about you, not the plan:
- Your doctors and hospital. Most Medicare Advantage plans use a network, and each hospital system here lists its own accepted plans. A plan that is excellent at Albany Med can be the wrong plan if your doctors admit to Saratoga Hospital. The hospital page has the lists side by side.
- Your prescriptions. Each plan has its own drug list, tiers and pharmacy network. The same three prescriptions can cost a few hundred dollars a year in one plan and a few thousand in another, so compare the total yearly drug cost, not the premium.
- How you want to pay. Original Medicare with Medigap costs more each month and little when you use care. Most Advantage plans cost little each month and charge copays when you use care, up to a yearly maximum. Which is "cheaper" depends on how much care you use.
The 2027 Medicare Advantage plans in each county
Medicare Advantage plans are filed county by county. These are the counts for 2027 from the CMS Medicare Advantage and Part D landscape file, contract year 2027, read October 27, 2026. Special Needs Plans are only open to people with Medicaid, certain chronic conditions, or living in a facility; the rest are open to anyone with Parts A and B who lives in the county.

Counts only, by design: Medicare's marketing rules don't allow a plan to be named next to its premium, benefits or star rating without filing, so the county pages list each plan by name and type and the comparison of costs happens with your own doctors and drugs in front of us. How to find the best plan for you, in five steps
- List your doctors and the hospital they admit to. If keeping any Medicare doctor anywhere matters most, that points to Original Medicare with Medigap. If your doctors are all local, check which Advantage plans include every one of them.
- List your prescriptions with doses and price them in each plan for the whole year, on Medicare's Plan Finder or with me. Premium plus drug costs is the number to compare.
- Set a budget for a bad year, not a good one. Add the yearly premiums to the plan's out-of-pocket maximum. That is what a hospital stay or a new diagnosis could cost you.
- Think about travel. Months in Florida or with family out of state favor Original Medicare with Medigap, or a PPO with out-of-network coverage.
- Check what you already have. NYSHIP, GE, union or other retiree coverage can conflict with an outside Advantage plan; Medicaid or Extra Help opens other options. Those come before any comparison.

What is the best Medicare plan in 2027?
There is no single best one. The best plan for you covers your doctors and hospital, covers your prescriptions at the lowest total cost for the year, and matches how you want to pay: more each month (Original Medicare with Medigap) or more when you use care (most Medicare Advantage plans). Check your own doctors and drug list against each plan in your county before you choose; the plan finder on this site narrows the field in eight questions.
Is Medicare Advantage or Medigap better?
Neither is better for everyone. Original Medicare with a Medigap plan lets you see any provider in the country who takes Medicare, with predictable costs and a higher monthly premium. Medicare Advantage usually costs less each month and adds extras, in exchange for a network and copays when you use care, up to a yearly out-of-pocket maximum. In New York you can buy a Medigap plan at any time without medical questions, which makes trying Advantage less of a one-way door than in most states.
What is the best Medicare plan in New York?
It depends on your county as much as on you. Medicare Advantage plans are filed by county, so Clifton Park (Saratoga County) and Latham (Albany County), ten minutes apart, have different line-ups, and each hospital system accepts different carriers. Start from the county and the hospital your doctors use; the county plan pages on this site list every plan filed for the year, by name and type.
Should I change my Medicare plan for 2027?
Only if this year's plan no longer fits. Read the Annual Notice of Change your plan mailed by September 30, check that your doctors are still in network and your drugs are still covered at the same tier, and compare the total yearly cost. If your plan is ending, you get a Special Enrollment Period. Most people keep their plan most years; the yearly check is what matters.
Who can tell me the best plan for my situation?
Medicare's own Plan Finder at medicare.gov shows every plan in your ZIP code. A licensed agent compares the plans they can offer against your doctors and prescriptions; I do that at no cost, and I tell you which plans I don't represent. HIICAP counselors at each county's Office for the Aging give free, unbiased help with every plan and enroll no one.
