Who they are

The Capital Region's own health plan, founded by local physicians in 1984 and still based here. CDPHP's provider network is built around Albany Med, St. Peter's, Ellis and Saratoga Hospital and the practices that admit to them, which is why it is the plan many Capital Region doctors' offices know best. Its Medicare Advantage plans are HMOs and PPOs sold in Albany, Saratoga, Schenectady and Rensselaer counties among others.

Plans are issued by Capital District Physicians' Health Plan, Inc., home office Albany, New York (registered office 500 Patroon Creek Boulevard, Albany; headquarters at 6 Wellness Way, Latham). Named here because New York Insurance Law §2122(b) requires a licensee who names an insurer to give its full name and home-office city.

Is CDPHP the right choice for me?
