Medicare agent for Little Falls

The City of Little Falls is in Herkimer County, and that decides which Medicare Advantage plans you can choose from. Little Falls Hospital, part of the Bassett Healthcare Network, is in the city on Burwell Street. My office is in Clifton Park, and I work with Little Falls residents by phone, WhatsApp and video.

Where Little Falls residents get care

The hospital and the Bassett practices on Burwell Street belong to one network, which contracts with Medicare Advantage carriers separately from the health system in Utica:

  • Little Falls Hospital, 140 Burwell Street (315-823-1000), part of the Bassett Healthcare Network, with 25 acute inpatient beds and an emergency department open 24 hours. Bassett also lists short-term inpatient rehabilitation, same-day surgery, imaging, dialysis and adult day health care there.
  • Bassett Health Center Little Falls Primary Care, 170 Burwell Street (315-823-4546, weekdays 8 to 5), with family primary care including elder care and care for chronic conditions.
  • Bassett Little Falls Specialty Services, at the hospital (315-823-4506, weekdays 8 to 4), with orthopedics, heart care, eye care, diabetes and kidney care, podiatry, surgery and other specialties. Bassett asks you to get a referral from your primary care practitioner first.
  • Bassett Medical Center in Cooperstown (607-547-3456) is the network's hub, and Wynn Hospital, 111 Hospital Drive in Utica (315-917-9966), is the Mohawk Valley Health System's hospital; MVHS lists Herkimer County in the area it serves. A plan that lists one system does not automatically list the other.

Bassett's insurance page says its practitioners take part in most of the health plans in the region, but that some plans offer specific products that leave Bassett practitioners out. With an HMO, your primary-care doctor, your specialists, the hospital and any rehab after a stay all have to be in the plan's directory for the year, and a specialist visit can need a referral under the plan as well as under the practice's own rules. Original Medicare with a Medigap policy has no network, so any provider that accepts Medicare works. None of these hospitals has a page of its own here; I check each hospital and doctor by name in the plan's directory.

What's particular about Little Falls

  • City or town, the same list. The City of Little Falls and the Town of Little Falls are separate municipalities, both in Herkimer County, so both see the Herkimer County plan list: 33 Medicare Advantage plans for 2027. Plans are filed by where you live, so seeing doctors in Utica or Cooperstown doesn't change your list; it changes which directories we have to check.
  • Getting to appointments. The Herkimer County Office for the Aging drives residents 60 and older anywhere in the county, including medical appointments, in wheelchair-accessible vans, but says it does not go to Utica or Cooperstown. Book at least a week ahead, or two to three weeks for a medical appointment (315-867-1121); there is a suggested contribution for each ride. If that's how you get around, the plan's doctors have to be somewhere those rides reach.
  • Medigap. In New York you can buy or switch Medigap in any month without health questions. Premiums are set by region, and I check the Department of Financial Services' published rates for yours.
  • Retiree and employer coverage first. State, school district and hospital retirees, and people still working at 65, have coverage that coordinates with Medicare in its own way. Whether an individual plan adds to it or cancels it comes before any comparison; NYSHIP retirees and Medicare while working past 65 cover the two most common cases.

How we work from Little Falls

My office is in Clifton Park, and I work with Little Falls residents by phone, WhatsApp and video. A first call takes about fifteen minutes: your ZIP code, your doctors and where they are, your prescriptions, and any retiree, VA or Medicaid coverage. Nothing is decided on a first call.

What a Little Falls plan review covers

  1. Every doctor you see, checked against each plan's current directory, including the hospital they use: Little Falls Hospital, Bassett Medical Center in Cooperstown or Wynn Hospital in Utica.
  2. Prescriptions priced through each plan's formulary for the full year, at the pharmacy you actually use.
  3. Retiree, employer, VA or Medicaid coverage and exactly how each option would interact with it, plus a screening for the Medicare Savings Program and Extra Help.
  4. A written comparison with total yearly cost, and a plain statement of whether your current coverage should be left alone.

Do Medicare Advantage plans in Little Falls include Little Falls Hospital?

Little Falls Hospital, the Bassett primary care center on Burwell Street and Bassett Little Falls Specialty Services are all part of the Bassett Healthcare Network. Bassett's participating-insurance page says its practitioners take part in all products for most of the health plans in the region, but that some plans offer specific products that don't include Bassett practitioners. Carriers are not plans, so before you enroll I check the hospital and each of your doctors by name in the specific plan's own directory.

I live in the Town of Little Falls, not the city. Is my plan list different?

No. The City of Little Falls and the Town of Little Falls are separate municipalities, but both are in Herkimer County, and Medicare Advantage and Part D plans are filed by county. Either address sees the Herkimer County plan list.

I don't drive. Can the county take me to appointments?

The Herkimer County Office for the Aging provides rides for county residents 60 and older, to medical appointments, shopping and other trips anywhere in the county, in wheelchair-accessible vans; another adult can come along if you need help. The county says it does not travel to Utica or Cooperstown. Call 315-867-1121 at least a week ahead, or two to three weeks ahead for a medical appointment; there is a suggested contribution for each ride. If your specialists are in Utica or Cooperstown, I look at how you'd get there before we choose a plan.

Where is free Medicare counselling for Little Falls residents?

The Herkimer County Office for the Aging, at 109 Mary Street, Suite 1101, in Herkimer, (315) 867-1121, contracts with Catholic Charities of Herkimer County to provide HIICAP, New York's Medicare counselling program, by appointment. The statewide HIICAP line is 1-800-701-0501. It's a good second opinion on anything I recommend.

Do you meet Little Falls clients in person?

No. My office is in Clifton Park, and I work with Little Falls residents by phone, WhatsApp and video. The comparison and the enrollment work the same way, and I stay your contact afterwards.
