Where Clifton Park residents get care

Every major Capital Region system has planted a flag along Route 146 and Sitterly Road, and each one admits to a different hospital:

• Ellis Medicine — Medical Center of Clifton Park, 103 Sitterly Road: emergent care seven days a week, primary care, imaging and lab. Hospital admissions go to Ellis Hospital at 1101 Nott Street in Schenectady.
• Albany Med Health System — the Clifton Park Specialty Care Center at 453 Route 146 and EmUrgentCare at 989 Route 146. Admissions go to Albany Medical Center at 43 New Scotland Avenue in Albany.
• St. Peter's Health Partners — the medical campus at 1 Tallow Wood Drive, with primary care and several specialties. Admissions go to St. Peter's Hospital at 315 South Manning Boulevard in Albany or Samaritan Hospital at 2215 Burdett Avenue in Troy.
• Saratoga Hospital — practices in Malta and Wilton, near the northern end of town; the hospital itself is at 211 Church Street in Saratoga Springs and is a member of the Albany Med Health System.

Most Clifton Park households see doctors from at least two of these. For a Medicare Advantage HMO that's a real constraint: a plan that includes your Ellis primary-care doctor may not include the Albany Med cardiologist she refers you to. Original Medicare with a Medigap policy has no network, so any doctor or hospital that accepts Medicare works. A PPO eases the problem without removing it: it can pay for providers outside its network, usually at a higher cost, so check the specific doctor, service and plan before relying on that flexibility. What's particular about Clifton Park

• Retiree coverage. If you retired from New York State, GE, a school district or another employer that provides retiree health coverage, whether an individual Medicare plan adds to that coverage or cancels it is the first question — before premiums, before extras.
• Two ZIP codes, one town. Clifton Park and Halfmoon share 12065; Rexford is 12148. Plan availability is set by county, so all of them see the Saratoga County line-up — but premiums for the same Medigap plan can differ from Albany County, ten minutes south.
• Winters away. If you spend part of the year in Florida or the Carolinas, check how a plan handles routine care away from home. An HMO generally covers care outside its network only for emergencies, urgent care away from home and temporary dialysis out of the area. A PPO may cover out-of-network providers there, usually at a higher cost; check the plan's rules for the provider and service first. Original Medicare with Medigap works with any provider in the country that accepts Medicare.
