• Saratoga Hospital and Albany Medical Center (Albany Med Health System). Saratoga Hospital's Primary Care Milton is at 510 Geyser Road, Ballston Spa (518-289-2720), weekdays 8 to 4:30, with preventive and sick visits, chronic condition care, behavioral health and lab services. The hospital itself is at 211 Church Street in Saratoga Springs (518-587-3222), and Albany Medical Center is at 43 New Scotland Avenue in Albany (518-262-3125).
• St. Peter's Health Partners. St. Peter's Primary Care – Ballston Spa is at 990 Route 67 (518-451-7520). St. Peter's Hospital is at 315 South Manning Boulevard in Albany (518-525-1550).
• Ellis Medicine. Ellis Hospital is at 1101 Nott Street in Schenectady (518-243-4000), and Ellis contracts with carriers on its own.

With a Medicare Advantage HMO, your primary-care doctor, your specialists and the hospital they admit to all have to be in the plan's directory for the year; Original Medicare with a Medigap policy has no network, so any provider that accepts Medicare works. What's particular about Ballston Spa

• The county's Medicare counselling is in the village. Ballston Spa is the county seat, and the Saratoga County Department of Aging and Youth Services, which runs HIICAP, is at 152 West High Street. If you want a second opinion on anything I recommend, it is a short trip rather than a phone call.
• Two systems in one town. The Saratoga Hospital practice on Geyser Road and the St. Peter's practice on Route 67 are in different health systems with different carrier lists. Neighbours who both say "my doctor is in Ballston Spa" can need different plans.
• Village, town, same plan list. Part of the Village of Ballston Spa lies in the Town of Milton, and Milton's town offices on Geyser Road use a Ballston Spa address too. None of that changes your plans: the 12020 ZIP area lies entirely in Saratoga County, so every Ballston Spa address sees the Saratoga County list: 52 Medicare Advantage plans for 2027. A move out of your plan's service area, or to an address with new plan options, opens a Special Enrollment Period that runs for two full months after the move.
• Medigap is priced like Albany.
