Medicare agent for Middleburgh
My office is in Clifton Park, and I work with Middleburgh residents by phone, WhatsApp and video. The Town and Village of Middleburgh are in Schoharie County, which has its own list of Medicare Advantage plans. Before anything else, I check how a plan treats the Bassett Health Center on Baker Avenue, Cobleskill Regional Hospital and the doctors you already see.
Where Middleburgh residents get care
- Bassett Health Center Middleburgh, 109 Baker Avenue (518-827-7730), a Bassett primary care center open weekdays 8 to 5. Its services include family medicine and physicals, chronic condition care, cancer screenings, routine blood tests and elder care.
- Cobleskill Regional Hospital, 178 Grandview Drive in Cobleskill (518-254-3456), a not-for-profit subsidiary of the Bassett Healthcare Network. Its emergency department is staffed around the clock (518-254-3337), and it has inpatient care for general medical conditions and a short-term rehabilitation unit.
Being in the same network does not make it the same check: I look up the health center, the hospital and each doctor separately. Bassett's participating-insurance page lists carriers, not individual Medicare Advantage plans, and says some health plans offer products where Bassett practitioners are not included. With a Medicare Advantage HMO, your primary-care doctor, your specialists, the hospital and any rehab stay all have to be in the plan's directory for the year. Original Medicare with a Medigap policy has no network, so any provider that accepts Medicare works. If you also see specialists in Schenectady or Albany, the pages for Ellis Hospital and Albany Medical Center show which carriers each one lists; I still check each hospital and doctor by name in the plan's directory.
What's particular about Middleburgh
- The Schoharie County list, wherever your doctors are. The Town and the Village of Middleburgh both see the Schoharie County plan list: 34 Medicare Advantage plans for 2027. Plans follow where you live, so a Schenectady cardiologist or an Albany specialist has to be in the directory of the Schoharie County plan you choose.
- Getting to appointments. Schoharie County Public Transportation (518-234-0952) is open to every county resident. Route 3 runs between Cobleskill, Central Bridge, Schoharie and Middleburgh on Wednesdays, Thursdays and Fridays, stopping at the Middleburgh Library; Route 20 goes to the Albany VA Hospital, Schenectady and the Capital Region on Mondays, Wednesdays and Fridays, with an on-call stop at the Middleburgh Public Library. Call reservations in by noon; there is no weekend dispatch. The Office for the Aging helps with bus fares by selling tokens to people 60 and over, and runs a limited volunteer medical-ride program for older adults who can't get there any other way.
- Medigap is priced like Albany. New York's Department of Financial Services compares Medigap premiums by region, and its Albany region covers ZIP codes starting 120–123 and 128–129, which takes in Middleburgh (12122). In New York you can buy or switch Medigap in any month without health questions; the current premiums for the region are on their own page.
- Social Security. Social Security's office locator assigns the 12122 ZIP code to its Schenectady office at One Broadway Center. Medicare Part A and B sign-up runs through Social Security.
- Retiree and employer coverage first. Whether an individual plan adds to retiree or employer coverage or cancels it comes before any comparison; NYSHIP retirees and Medicare while working past 65 cover the two most common cases.
Schoharie County Office for the Aging
The Schoharie County Office for the Aging, 113 Park Place, Suite 3, in Schoharie (518-295-2001, weekdays 8 to 5), has a HIICAP coordinator who offers New York's free and unbiased Medicare counselling, including help with Medicare forms, medical bills and Part D enrollment. It also sells bus tokens to people 60 and over. Help with costs: Medicare Savings Program, Extra Help and EPIC. The HIICAP page has every county office. Statewide HIICAP line: 1-800-701-0501.

How we work from Middleburgh
My office is in Clifton Park, and I work with Middleburgh residents by phone, WhatsApp and video. A first call takes about fifteen minutes: your ZIP code, your doctors and where they are, your prescriptions, and any retiree, VA or Medicaid coverage. Nothing is decided on a first call.
What a Middleburgh plan review covers
- Every doctor you see, checked against each plan's current directory, including the Bassett Health Center on Baker Avenue, Cobleskill Regional and any specialists in Schenectady or Albany.
- Prescriptions priced through each plan's formulary for the full year, at the pharmacy you actually use.
- Retiree, employer, VA or Medicaid coverage and exactly how each option would interact with it, plus a screening for the Medicare Savings Program and Extra Help.
- A written comparison with total yearly cost, and a plain statement of whether your current coverage should be left alone.

Still have a question? Call (518) 902-8822 or book a call.
Do Medicare Advantage plans in Middleburgh include the Bassett Health Center and Cobleskill Regional Hospital?
It depends on the plan. The Bassett Health Center in Middleburgh and Cobleskill Regional Hospital are both part of the Bassett Healthcare Network. Bassett's participating-insurance page has a Medicare Advantage section listing carriers, advises patients to verify benefits directly with their plan, and says some health plans offer products where Bassett practitioners are not included. A carrier is not a plan, so before you enroll I check the health center, the hospital and each of your doctors by name in the specific plan's own directory.
My doctor is at the Bassett Health Center on Baker Avenue. Is that a separate check from the hospital?
Yes. The health center at 109 Baker Avenue is a primary care site, and Cobleskill Regional Hospital at 178 Grandview Drive in Cobleskill is a hospital. I look up both by name in the plan's directory, along with any specialist you see elsewhere. Your plan list stays Schoharie County's, because plans follow where you live, not where your doctors are.
I don't drive. How do I get to medical appointments from Middleburgh?

Call reservations in by noon; there is no weekend dispatch. The Office for the Aging sells bus tokens to people 60 and over and has a limited volunteer medical-ride program for older adults with no other way to get there (518-295-2001).
Where is free Medicare counselling for Middleburgh residents?
