Medicare agent for Fort Ann

Fort Ann is a town and village in Washington County, named for the Battle of Fort Anne fought there in 1777, with Route 4 running through the village between Hudson Falls and Whitehall. Neither Glens Falls Hospital nor Hudson Headwaters lists a practice in Fort Ann, so the first thing a Medicare plan has to fit is the practices and hospital you travel to.

Where Fort Ann residents get care

  • Glens Falls Hospital and its practices. The hospital is at 100 Park Street in Glens Falls (518-926-1000), a member of the Albany Med Health System with an emergency department open around the clock. Its list of physician practices has none in Fort Ann; the Hudson Falls Medical Center at 325 Main Street in Hudson Falls (518-747-3376) is where Glens Falls Hospital offers primary care and geriatric medicine on weekdays. Albany Medical Center, at 43 New Scotland Avenue in Albany (518-262-3125), is in the same system, but a shared system does not mean every practice has the same plan contracts.
  • Hudson Headwaters. Hudson Headwaters Health Network has no location in Fort Ann; its Washington County health centers are the Fort Edward-Kingsbury Health Center at 48 East Street in Fort Edward and Salem Family Health at 1830 County Route 64 in Salem.
  • Saratoga, Albany and Vermont. If your doctors are in Saratoga Springs or Albany, check them separately. If you use doctors or a hospital across the state line, Original Medicare with Medigap works with any provider that accepts Medicare; a Medicare Advantage plan works only as far as its network, so each Vermont provider has to be checked by name.

With a Medicare Advantage HMO, out-of-network care is generally covered only for emergencies, out-of-area urgent care and temporary dialysis; a PPO generally lets you go out of network at a higher cost. Original Medicare with a Medigap policy has no network at all. The Glens Falls Hospital page places every plan sold in Washington County against the hospital's own list, and the carriers for every Capital Region hospital are side by side on one page; I still check each hospital and doctor by name in the plan's directory.

What's particular about Fort Ann

  • Care is out of town. Neither Glens Falls Hospital's practice list nor Hudson Headwaters' locations page shows a practice in Fort Ann, so every doctor you see is somewhere else: Hudson Falls, Fort Edward, Glens Falls or elsewhere. Each one, and the hospital they admit to, has to be in a Medicare Advantage plan's directory for the year.
  • Town and village, one county list. Town Hall is at 80 George Street and Village Hall at 67 Anne Street, and the 12827 ZIP area lies entirely in Washington County, which has 44 Medicare Advantage plans for 2027. A move out of your plan's service area, or to an address with new plan options, opens a Special Enrollment Period that runs for two full months after the move.
  • A town rescue squad. The town lists the Fort Ann Rescue Squad at 11287 State Route 149, (518) 639-1005. How a plan handles ambulance and emergency care is one of the lines I put side by side.
  • Medigap is priced like Albany. New York's Department of Financial Services compares Medigap premiums by region, and its Albany region covers ZIP codes starting 120–123 and 128–129, which takes in Fort Ann (12827). In New York you can buy or switch Medigap in any month without health questions; the current premiums for the region are on their own page.
  • Retiree and employer coverage first. State, county, school district and hospital retirees, and people still working at 65, have coverage that coordinates with Medicare in its own way. Whether an individual plan adds to it or cancels it comes before any comparison; NYSHIP retirees and Medicare while working past 65 cover the two most common cases.

Meeting in Fort Ann

My office is in Clifton Park, off Northway Exit 9. I come to Fort Ann clients at home, in the town or the village, or we talk by phone, WhatsApp or video. A first call takes about fifteen minutes: your ZIP code, your doctors and where they are, your prescriptions, and any retiree, VA or Medicaid coverage. Nothing is decided on a first call.

What a Fort Ann plan review covers

  1. Every doctor you see, checked against each plan's current directory: the Hudson Falls practice or wherever your own doctor is, Glens Falls Hospital, and any Vermont providers.
  2. Prescriptions priced through each plan's formulary for the full year, at the pharmacy you actually use.
  3. Retiree, VA or Medicaid coverage and exactly how each option would interact with it, plus a screening for the Medicare Savings Program and Extra Help.
  4. A written comparison with total yearly cost, and a plain statement of whether your current coverage should be left alone.

There's no doctor's office in Fort Ann on the hospital's list. Which practices do plans here need to include?

The ones you actually use. Glens Falls Hospital's list of physician practices, which says it serves Warren, Washington, Essex, Hamilton and northern Saratoga counties, has none in Fort Ann; it does include the Hudson Falls Medical Center at 325 Main Street, with primary care and geriatric medicine. Hudson Headwaters Health Network's Washington County health centers are in Fort Edward and Salem. A carrier's name on a hospital's insurance list does not mean every plan from that carrier is in network, so I check each practice, your own doctor and the hospital by name in the plan's directory for the year.

My doctors are in Vermont. Can a New York Medicare plan cover them?

Original Medicare with a Medigap policy covers any provider in the country that accepts Medicare, including Vermont. A Medicare Advantage plan depends on its network: an HMO generally covers out-of-network care only for emergencies, urgent care away from home and temporary out-of-area dialysis, and a PPO generally lets you go out of network at a higher cost. Check each Vermont doctor and hospital by name in the plan's own directory before you enroll.

Do the Town and Village of Fort Ann have the same Medicare plans?

Yes. Both are in Washington County, and Medicare Advantage and Part D plans are filed by county. The Census Bureau puts all of the land in the Fort Ann 12827 ZIP area in Washington County, so every Fort Ann address sees the same Washington County plan list. What differs from one person to the next is which plans include your doctors and price your prescriptions well.

Where is HIICAP Medicare counselling for Fort Ann residents?

HIICAP, New York's Medicare counselling program, has trained counsellors in local offices for the aging. In Washington County that is the Office for the Aging & Disability Resource Center at 383 Broadway in Fort Edward, (518) 746-2420, and the statewide HIICAP line is 1-800-701-0501. I enroll you in the plans I'm appointed with and stay your contact afterwards; HIICAP is the second opinion I recommend to anyone who wants one.

Do you meet Fort Ann clients in person?

Yes. My office is in Clifton Park, off Northway Exit 9, and I come to you at home in the town or village of Fort Ann. We can also talk by phone, WhatsApp or video. No fee for Medicare plan help.
