Medicare agent for Canajoharie
My office is in Clifton Park, and I work with Canajoharie residents by phone, WhatsApp and video. The Town and the Village of Canajoharie are in Montgomery County, and that decides which Medicare Advantage plans you can choose from. Before anything else, I check how a plan treats the two health centers in the village and the doctors you already see.
Where Canajoharie residents get care
The village has two health centers, run by two separate health systems that contract with Medicare Advantage carriers on their own:
- Bassett Health Center Canajoharie, 56 Montgomery Street (518-673-5555, weekdays 8 to 5), part of the Bassett Healthcare Network, with family medicine including senior care, chronic condition care, routine blood tests and screenings. Bassett's hospitals include Little Falls Hospital, 140 Burwell Street in Little Falls (315-823-1000), with a 24-hour emergency department, and Bassett Medical Center, 1 Atwell Road in Cooperstown (607-547-3456), a 180-bed teaching hospital.
- Canajoharie Family Health Center, 48 Erie Boulevard (518-673-2573, weekdays 7:30 to 5), part of St. Mary's Healthcare, with primary care, chronic care management, telehealth and a lab (weekdays 7:30 to 3, closed 12 to 1). St. Mary's also runs the St. Johnsville Urgent Care Center at 7 Timmerman Avenue in St. Johnsville (518-770-7818), and its hospital is St. Mary's Hospital, 427 Guy Park Avenue in Amsterdam (518-842-1900).
Bassett's insurance page says its practitioners take part in most of the health plans in the region, but that some plans offer specific products that leave them out; St. Mary's accepted-insurance page lists carriers, not Medicare Advantage plans. With an HMO, your primary-care doctor, your specialists, the hospital and any rehab after a stay all have to be in the plan's directory for the year. Original Medicare with a Medigap policy has no network, so any provider that accepts Medicare works. If you also see specialists in Schenectady or Albany, the pages for Ellis Hospital and Albany Medical Center show which carriers each one lists; I still check each hospital and doctor by name in the plan's directory.
What's particular about Canajoharie
- Town, village and county lines. The Village of Canajoharie sits inside the Town of Canajoharie, where Canajoharie Creek meets the Mohawk, and the Village of Ames is in the town too. All of it is Montgomery County, which has 39 Medicare Advantage plans for 2027. The town's southern border is the county line: Otsego County (28 plans) and Schoharie County (34 plans) have their own lists. Plans follow the county you live in, not where your doctors are.
- Two networks in one village. The Bassett center belongs to the network with Little Falls Hospital and Bassett Medical Center in Cooperstown; the Family Health Center belongs to St. Mary's, whose hospital is in Amsterdam. A plan that lists one system does not automatically list the other, so the first question is which system your doctors belong to.
- Getting to appointments. The Office for the Aging's OFA Senior Transport gives rides to medical appointments, lab work, dental and eye visits and outpatient procedures for Montgomery County residents 60 and over who can manage getting in and out of the car on their own and aren't Medicaid eligible for medical transport. Send the paperwork first, then book at least seven days ahead (518-843-2300); rides depend on driver availability. Taxi tickets can cover part, and sometimes all, of a ride to a medical appointment. If that's how you get around, the plan's doctors have to be somewhere those rides reach.
- Medigap. In New York you can buy or switch Medigap in any month without health questions. Premiums are set by region, and I check the Department of Financial Services' published rates for yours.
- Retiree and employer coverage first. State, school district and hospital retirees, and people still working at 65, have coverage that coordinates with Medicare in its own way. Whether an individual plan adds to it or cancels it comes before any comparison; NYSHIP retirees and Medicare while working past 65 cover the two most common cases.

I live in the Town of Canajoharie, outside the village. Is my plan list different?
No. The Village of Canajoharie lies inside the Town of Canajoharie, along with the Village of Ames, and both villages and the town are in Montgomery County. Medicare Advantage and Part D plans are filed by county, so any of those addresses sees the Montgomery County plan list. The town borders Otsego and Schoharie counties to the south, and addresses across that line see those counties' lists instead.
Do Medicare Advantage plans in Canajoharie include the Bassett and St. Mary's health centers?
Bassett Health Center Canajoharie on Montgomery Street is part of the Bassett Healthcare Network, and the Canajoharie Family Health Center on Erie Boulevard is part of St. Mary's Healthcare in Amsterdam. Bassett's insurance page says its practitioners take part in most of the health plans in the region, but that some plans offer specific products that leave them out. St. Mary's accepted-insurance page lists carriers, not Medicare Advantage plans. A carrier is not a plan, so before you enroll I check each center and each of your doctors by name in the specific plan's own directory.
I don't drive. How do I get to medical appointments from Canajoharie?
The Montgomery County Office for the Aging runs OFA Senior Transport, rides to medical appointments, lab work, dental and eye visits and outpatient procedures for county residents 60 and over who can get in and out of the car on their own and aren't Medicaid eligible for medical transport. The paperwork has to be in first, and rides need at least seven days' notice and depend on driver availability; call 518-843-2300. The office also has taxi tickets that pay part, and sometimes all, of a ride to a medical appointment for people 60 and over.
Where is free Medicare counselling for Canajoharie residents?
The Montgomery County Office for the Aging, Inc., at 135 Guy Park Avenue in Amsterdam, (518) 843-2300, runs HIICAP, New York's free and unbiased Medicare counselling program, by appointment. It covers Medicare Parts A through D, Medigap, EPIC, the low-income subsidy and help with claims. The office also has a Canajoharie location at 50 Front Street, on the same phone number. The statewide HIICAP line is 1-800-701-0501.
Do you meet Canajoharie clients in person?
No. My office is in Clifton Park, and I work with Canajoharie residents by phone, WhatsApp and video. The comparison and the enrollment work the same way, and I stay your contact afterwards.
