Medicare agent for Fort Plain

My office is in Clifton Park, and I work with Fort Plain residents by phone, WhatsApp and video. The Village of Fort Plain lies in the Town of Minden, in Montgomery County, and the county decides which Medicare Advantage plans you can choose from. Before anything else, I check how a plan treats the Bassett and St. Mary's doctors you already see.

Where Fort Plain residents get care

Two health systems run the health centers and hospitals around Fort Plain, and each contracts with Medicare Advantage carriers on its own. The Town of Canajoharie, the next town to the east, has a health center from each:

  • Bassett Healthcare Network. Bassett Health Center Canajoharie, 56 Montgomery Street in Canajoharie (518-673-5555, weekdays 8 to 5), with family medicine including senior care, chronic condition care, routine blood tests and screenings. Bassett's hospitals include Little Falls Hospital, 140 Burwell Street in Little Falls (315-823-1000), with a 24-hour emergency department, short-term rehabilitation and dialysis, and Bassett Medical Center, 1 Atwell Road in Cooperstown (607-547-3456), a 180-bed teaching hospital.
  • St. Mary's Healthcare. The Canajoharie Family Health Center, 48 Erie Boulevard in Canajoharie (518-673-2573), the St. Johnsville Urgent Care Center, 7 Timmerman Avenue in St. Johnsville (518-770-7818), and St. Mary's Hospital, 427 Guy Park Avenue in Amsterdam (518-842-1900).

Bassett's insurance page says its practitioners take part in most of the health plans in the region, but that some plans offer specific products that leave them out. With an HMO, your primary-care doctor, your specialists, the hospital and any rehab after a stay all have to be in the plan's directory for the year. Original Medicare with a Medigap policy has no network, so any provider that accepts Medicare works. If you also see specialists in Schenectady or Albany, the pages for Ellis Hospital and Albany Medical Center show which carriers each one lists; I still check each hospital and doctor by name in the plan's directory.

What's particular about Fort Plain

  • Village, town and county lines. The County's hazard plan places the Village of Fort Plain within the Town of Minden, where Otsquago Creek meets the Mohawk. The village and the town are both Montgomery County, which has 39 Medicare Advantage plans for 2027. Minden's southern border is the Otsego County line (28 plans) and its western border the Herkimer County line (33 plans), and each county has its own list. Plans follow the county you live in, not where your doctors are.
  • Two networks, two directions. Bassett's hospitals are in Little Falls and Cooperstown; St. Mary's hospital is in Amsterdam. A plan that lists one system does not automatically list the other, so the first question is which system your doctors belong to.
  • Getting to appointments. The Office for the Aging's OFA Senior Transport uses volunteer drivers to take Montgomery County residents 60 and over to non-emergency medical appointments, local and out of town, if they can get in and out of the car on their own and aren't Medicaid eligible for medical transport. Mail the paperwork first, then book at least seven days ahead (518-843-2300); rides depend on driver availability, and you pay any parking or tolls. Taxi tickets can cover part, and sometimes all, of a ride to a medical appointment. If that's how you get around, the plan's doctors have to be somewhere those rides reach.
  • Medigap. In New York you can buy or switch Medigap in any month without health questions. Premiums are set by region, and I check the Department of Financial Services' published rates for yours.
  • Retiree and employer coverage first. State, school district and hospital retirees, and people still working at 65, have coverage that coordinates with Medicare in its own way. Whether an individual plan adds to it or cancels it comes before any comparison; NYSHIP retirees and Medicare while working past 65 cover the two most common cases.

How we work from Fort Plain

My office is in Clifton Park, and I work with Fort Plain residents by phone, WhatsApp and video. A first call takes about fifteen minutes: your ZIP code, your doctors and where they are, your prescriptions, and any retiree, VA or Medicaid coverage. Nothing is decided on a first call.

What a Fort Plain plan review covers

  1. Every doctor you see, checked against each plan's current directory, including the Bassett and St. Mary's centers in Canajoharie, the St. Johnsville Urgent Care Center and the hospital your doctors use: Little Falls Hospital, Bassett Medical Center or St. Mary's in Amsterdam.
  2. Prescriptions priced through each plan's formulary for the full year, at the pharmacy you actually use.
  3. Retiree, employer, VA or Medicaid coverage and exactly how each option would interact with it, plus a screening for the Medicare Savings Program and Extra Help.
  4. A written comparison with total yearly cost, and a plain statement of whether your current coverage should be left alone.

Questions

Common questions

Short answers to what people ask before they call.

Still have a question? Call (518) 902-8822 or book a call.

I live in the Town of Minden, outside the village. Is my plan list different?

No. The County's hazard mitigation plan places the Village of Fort Plain within the Town of Minden, and the state's county profile lists the village under Minden. Both are in Montgomery County, and Medicare Advantage and Part D plans are filed by county, so any of those addresses sees the Montgomery County plan list. The town borders Otsego County to the south and Herkimer County to the west, and addresses across those lines see those counties' lists instead.

Do Medicare Advantage plans for Fort Plain include Bassett and St. Mary's?

Bassett Health Center Canajoharie on Montgomery Street and Little Falls Hospital are part of the Bassett Healthcare Network; the Canajoharie Family Health Center, the St. Johnsville Urgent Care Center and St. Mary's Hospital in Amsterdam are part of St. Mary's Healthcare. Bassett's insurance page says its practitioners take part in most of the health plans in the region, but that some plans offer specific products that leave them out. A carrier is not a plan, so before you enroll I check each center and each of your doctors by name in the specific plan's own directory.

I don't drive. How do I get to medical appointments from Fort Plain?

The Montgomery County Office for the Aging runs OFA Senior Transport, with volunteer drivers taking county residents 60 and over to non-emergency medical appointments, local and out of town, including office visits, lab work, dental and eye care and outpatient procedures. You have to be able to get in and out of your home and the car on your own and not be Medicaid eligible for medical transport. Mail the paperwork first, then book at least seven days ahead (518-843-2300); rides depend on driver availability, and you pay any parking or tolls. The office also has taxi tickets that cover part, and sometimes all, of a ride to a medical appointment for people 60 and over.

Where is free Medicare counselling for Fort Plain residents?

The Montgomery County Office for the Aging, Inc., at 135 Guy Park Avenue in Amsterdam, (518) 843-2300, runs HIICAP, New York's free and unbiased Medicare counselling program, by appointment. It covers Medicare Parts A through D, Medigap, EPIC, the low-income subsidy and help with claims. The office also has a satellite location at 50 Front Street in Canajoharie, on the same phone number. The statewide HIICAP line is 1-800-701-0501.

Do you meet Fort Plain clients in person?

No. My office is in Clifton Park, and I work with Fort Plain residents by phone, WhatsApp and video. The comparison and the enrollment work the same way, and I stay your contact afterwards.
