Medicare agent for Johnstown
My office is in Clifton Park, and I work with Johnstown residents by phone, WhatsApp and video. The City of Johnstown is in Fulton County, which has its own list of Medicare Advantage plans; the county's Office for Aging and Youth and its Department of Social Services are both in the city, and two separate health organizations have care centers on South Perry Street.
Where Johnstown residents get care
Two separate organizations have care centers on South Perry Street, each tied to its own hospital:
- Nathan Littauer Hospital & Nursing Home — the Johnstown Primary/Specialty Care Center at 23 South Perry Street (518-736-1500, weekdays 8 to 5), with internal medicine, family practice and lab services. The hospital is at 99 East State Street in Gloversville (518-725-8621): a 74-bed acute care hospital with an 84-bed skilled nursing home.
- St. Mary's Healthcare — the Johnstown Family Health Center at 700 South Perry Street (518-762-3161). Its hospital is St. Mary's Hospital, 427 Guy Park Avenue in Amsterdam (518-842-1900).
Littauer's billing page says physicians who work in the hospital may or may not take the same plans as the hospital, and that contracted physicians negotiate their own insurance contracts, so the hospital and each doctor get checked separately. With a Medicare Advantage HMO, your primary-care doctor, your specialists, the hospital and any rehab or nursing stay all have to be in the plan's directory for the year. Original Medicare with a Medigap policy has no network, so any provider that accepts Medicare works. If you also see specialists further away, the pages for Saratoga Hospital, Ellis Hospital and Albany Medical Center show which carriers each one lists; I still check each hospital and doctor by name in the plan's directory.
What's particular about Johnstown
- The county offices are in the city. The Fulton County Office for Aging and Youth is at 19 North William Street (518-736-5650), with insurance counselling by appointment, and the Department of Social Services is at 4 Daisy Lane (518-736-5600), listing Medicaid and Medicaid transportation among its services. If you may qualify for the Medicare Savings Program, Extra Help or EPIC, that screening comes before any plan comparison, because it changes what every plan costs you.
- Two hospitals, two counties, one plan list. Johnstown addresses see the Fulton County list: 37 Medicare Advantage plans for 2027. Nathan Littauer is in Gloversville, in the same county; St. Mary's Hospital is in Amsterdam, in Montgomery County (39 plans). Plans follow where you live, so what matters is whether the hospital your doctor uses is in the directory of the Fulton County plan you pick.
- Getting to appointments. The Gloversville Transit System's Route 2 runs through Johnstown on weekdays, stopping at Johnstown Four Corners, the county Department of Social Services and a store pharmacy, with a reduced senior fare and paratransit by application (518-773-4528). Its route list names no hospital or health center stop, so check how you would reach your doctors. The county's door-to-door, wheelchair-accessible bus for people 60 and over runs countywide; reserve at least 24 hours ahead at 518-736-5650.
- Medigap is priced like Albany. New York's Department of Financial Services compares Medigap premiums by region, and its Albany region covers ZIP codes starting 120–123 and 128–129, which takes in Johnstown (12095). In New York you can buy or switch Medigap in any month without health questions; the current premiums for the region are on their own page.
- Social Security is next door in Gloversville. Social Security's office locator assigns the 12095 ZIP code to its Gloversville office at 13 North Arlington Avenue. Medicare Part A and B sign-up runs through Social Security.
- Retiree and employer coverage first. Whether an individual plan adds to retiree or employer coverage or cancels it comes before any comparison; NYSHIP retirees and Medicare while working past 65 cover the two most common cases.
The Shirley J. Luck Center and the Fulton County Office for Aging and Youth
The City's Shirley J. Luck Center for 50+, at 109 East Main Street (518-762-4643), is open Monday to Thursday 8 to 3 and Friday mornings, with exercise classes, computer classes, card games and a blood pressure clinic on the third Thursday of each month; it welcomes people from outside the city too. The county lists no senior meal site in Johnstown; its weekday lunch is served at Forest Hill Towers in Gloversville, reserved through the Fulton County Office for Aging and Youth, 19 North William Street (518-736-5650), which also offers counselling on supplemental insurance by appointment. For HIICAP, New York's free and unbiased Medicare counselling, call the statewide line, 1-800-701-0501. The HIICAP page has every county office.

How we work from Johnstown
My office is in Clifton Park, and I work with Johnstown residents by phone, WhatsApp and video. A first call takes about fifteen minutes: your ZIP code, your doctors and where they are, your prescriptions, and any retiree, VA or Medicaid coverage. Nothing is decided on a first call.
What a Johnstown plan review covers
- Every doctor you see, checked against each plan's current directory, including whether they are with Nathan Littauer or St. Mary's and which hospital they admit to.
- Prescriptions priced through each plan's formulary for the full year, at the pharmacy you actually use.
- Retiree, employer, VA or Medicaid coverage and exactly how each option would interact with it, plus a screening for the Medicare Savings Program and Extra Help.
- A written comparison with total yearly cost, and a plain statement of whether your current coverage should be left alone.

My doctor is at a health center on South Perry Street. Does it matter which one?
Yes. There are two, run by different organizations. Nathan Littauer's Johnstown Primary/Specialty Care Center is at 23 South Perry Street, with internal medicine, family practice and lab services; Littauer's hospital is in Gloversville. St. Mary's Healthcare runs the Johnstown Family Health Center at 700 South Perry Street; its hospital is St. Mary's in Amsterdam. A Medicare Advantage plan can include one organization and not the other, so I check your doctor, the center and the hospital each one admits to by name in the plan's own directory.
