Medicare agent for Windham
My office is in Clifton Park, and I work with Windham residents by phone, WhatsApp and video. The Town of Windham, with Hensonville, East Windham, North Settlement, Maplecrest and Brooksburg, is in Greene County, and the county decides which Medicare Advantage plans you can choose from, wherever your doctors are.
Where Windham residents get care
- Windham Medical Care, 345 State Route 296, Hensonville (518-734-3260) — a family medicine and primary care practice, open weekdays 7:30 to 4:30, that Columbia Memorial Health lists among its physician practices.
- Columbia Memorial Health, part of the Albany Med Health System, with care centers across Columbia and Greene counties. Its hospital is at 71 Prospect Avenue in Hudson (518-828-7601), in Columbia County. The system's accepted-insurance page gives Columbia Memorial Hospital its own list of participating plans, separate from Albany Medical Center's.
- Doctors in Albany. If your doctors are in Albany, check them separately: being in the same health system does not mean the same plan contracts. The Albany Medical Center page sets out that hospital's own accepted-insurance list.
With a Medicare Advantage HMO, your primary-care doctor, your specialists, the hospital they admit to and, after a hospital stay, the rehab or nursing facility all have to be in the plan's directory for the year. Original Medicare with a Medigap policy has no network, so any provider that accepts Medicare works. Columbia Memorial has no page of its own here; I check the hospital, the Hensonville practice and each of your doctors by name in the plan's directory.
What's particular about Windham
- Your hospital can be in another county. Windham sees the Greene County plan list: 49 Medicare Advantage plans for 2027. Hudson, where Columbia Memorial's hospital is, is in Columbia County (48 plans). A Greene County plan has to list the Hudson hospital for it to be in network. A move out of your plan's service area, or to an address with new plan options, opens a Special Enrollment Period that runs for two full months after the move.
- Getting to appointments. Greene County Transit's Route 708 (Wednesdays) and Route 709 (Fridays) both stop at the Windham post office and run to Catskill; Route 709 also stops at the Windham Fire Department on Main Street and at the medical offices on Jefferson Heights in Catskill. Request-only stops need a call to Greene County Transit the day before, during business hours. Riders 60 and over ride fully subsidized after registering with the Greene County Department of Human Services (518-719-3555), which also lists homebound medical transportation and a door-to-door shopping bus.
- The ambulance and the pharmacy. The Town of Windham runs its own ambulance department, with a billing clerk at the Town (518-734-5574). Medicare Advantage plans set their own cost for ambulance trips, so I show it for each plan. The county's directory of services lists a pharmacy in Windham; I price your prescriptions at the pharmacy you actually use.
- Medigap. In New York you can buy or switch Medigap in any month without health questions. Premiums are set by region, and I check the Department of Financial Services' published rates for yours.
- Retiree coverage and part of the year away. If you have retiree coverage, how it coordinates with Medicare comes first; NYSHIP retirees and Medicare while working past 65 cover the two most common cases. If you spend part of the year somewhere else, an HMO generally covers care outside its network only for emergencies, urgent care away from home and temporary out-of-area dialysis; Original Medicare with Medigap works with any provider in the country that accepts Medicare.

The same office handles registration for subsidized bus fares at 60 and over, homebound medical transportation and the shopping bus. The HIICAP page has every county office. My doctor is at Windham Medical Care in Hensonville. Do Medicare Advantage plans include it?
It depends on the plan. Windham Medical Care, at 345 State Route 296 in Hensonville, is a family medicine and primary care practice that Columbia Memorial Health lists among its physician practices, part of the Albany Med Health System. The system's accepted-insurance page gives Columbia Memorial Hospital its own list of participating plans, separate from Albany Medical Center's. I check the practice, the hospital its doctors use and each of your other doctors by name in the specific plan's directory.
I live in Windham and my hospital is in Hudson. Which plan list do I get?
Greene County's. Medicare Advantage and Part D plans are filed by county and follow where you live, not where your doctors are. Windham is in Greene County; Hudson, where Columbia Memorial's hospital is, is in Columbia County, which has its own list. What matters for the hospital and your doctors is whether they are in the directory of the Greene County plan you choose, and I check each one by name.
I don't drive. Can I get from Windham to appointments in Catskill?
Greene County Transit runs two routes that stop in Windham and go to Catskill: Route 708 on Wednesdays and Route 709 on Fridays, which also stops at the medical offices on Jefferson Heights in Catskill. Some stops are on request only, and for those you call Greene County Transit the day before, during business hours. Riders 60 and over can ride fully subsidized after registering with the Greene County Department of Human Services (518-719-3555). When you rely on the bus, I check that a plan's doctors are somewhere you can reach.
