Medicare agent for Coxsackie
My office is in Clifton Park, and I work with Coxsackie residents by phone, WhatsApp and video. The Town and Village of Coxsackie are on the west side of the Hudson in Greene County, and the county decides which Medicare Advantage plans you can choose from, wherever your doctors are.
Where Coxsackie residents get care
- Lafayette Family Care at 24 Lafayette Avenue in Coxsackie (518-731-7777; weekdays, 8 to 4), with family medicine, internal medicine and primary care. It is part of the Albany Med Health System, and Columbia Memorial Health lists it among its physician practices.
- Columbia Memorial Health, part of the Albany Med Health System, with care centers across Columbia and Greene counties. Its hospital is across the river at 71 Prospect Avenue in Hudson (518-828-7601). The system's accepted-insurance page gives Columbia Memorial Hospital its own list of participating plans, by carrier.
- Albany Medical Center at 43 New Scotland Avenue in Albany (518-262-3125), which has a separate list on the same page. Being in the same health system does not mean the same plan contracts. The Albany Medical Center page sets out which carriers it lists.
With a Medicare Advantage HMO, your primary-care doctor, your specialists, the hospital they admit to and, after a hospital stay, the rehab or nursing facility all have to be in the plan's directory for the year. Original Medicare with a Medigap policy has no network, so any provider that accepts Medicare works. Columbia Memorial has no page of its own here; I check the hospital, Lafayette Family Care and each of your doctors by name in the plan's directory.
What's particular about Coxsackie
- Your doctors can be in another county. Coxsackie sees the Greene County plan list: 49 Medicare Advantage plans for 2027. Albany, where Albany Medical Center is, is in Albany County (57 plans); Hudson, where Columbia Memorial's hospital is, is in Columbia County (48 plans). A Greene County plan has to list those hospitals for them to be in network. A move out of your plan's service area, or to an address with new plan options, opens a Special Enrollment Period that runs for two full months after the move.
- Getting to appointments. Greene County's aging services list homebound medical transportation and a door-to-door shopping bus for older residents, alongside Greene County Transit; ask the Department for the Aging (518-719-3555) what runs from your address. If that's how you get around, the plan's doctors have to be somewhere you can reach.
- Medigap is priced like Albany. New York's Department of Financial Services compares Medigap premiums by region, and its Albany region covers ZIP codes starting 120–123 and 128–129, which takes in Coxsackie (12051). In New York you can buy or switch Medigap in any month without health questions; the current premiums for the region are on their own page.
- Retiree and employer coverage first. State, school district and hospital retirees, and people still working at 65, have coverage that coordinates with Medicare in its own way. Whether an individual plan adds to it or cancels it comes before any comparison; NYSHIP retirees and Medicare while working past 65 cover those two cases.
Coxsackie Senior Center and the Greene County Department for the Aging
The Town of Coxsackie says senior lunches are served at the Coxsackie Senior Center for adults 60 and over, weekdays at 11:45, except on holidays or when schools close for bad weather; Greene County lists Coxsackie among its senior nutrition sites. Free, unbiased Medicare counselling is through the Greene County Department for the Aging at 411 Main Street in Catskill (518-719-3555), which offers HIICAP by appointment and helps you apply for EPIC, New York's prescription help. The HIICAP page has every county office. Statewide HIICAP line: 1-800-701-0501.

How we work from Coxsackie
My office is in Clifton Park, and I work with Coxsackie residents by phone, WhatsApp and video. A first call takes about fifteen minutes: your ZIP code, your doctors and where they are, your prescriptions, and any retiree, VA or Medicaid coverage. Nothing is decided on a first call.
What a Coxsackie plan review covers
- Every doctor you see, checked against each plan's current directory, including Lafayette Family Care, Columbia Memorial's hospital in Hudson and any doctors in Albany.
- Prescriptions priced through each plan's formulary for the full year, at the pharmacy you actually use.
- Retiree, employer, VA or Medicaid coverage and exactly how each option would interact with it, plus a screening for the Medicare Savings Program and Extra Help.
- A written comparison with total yearly cost, gone through together by video or phone, and a plain statement of whether your current coverage should be left alone.

Short answers to what people ask before they call.
Still have a question? Call (518) 902-8822 or book a call.
I live in Coxsackie and my doctors are in Albany. Which plan list do I get?
Greene County's. Medicare Advantage and Part D plans are filed by county and follow where you live, not where your doctors are. Coxsackie is in Greene County; Albany is in Albany County and Hudson is in Columbia County, and each has its own list. What matters for your Albany or Hudson doctors is whether they are in the directory of the Greene County plan you choose, and I check each one by name.
Do Medicare Advantage plans in Coxsackie include my doctor at Lafayette Family Care?
It depends on the plan. Lafayette Family Care, at 24 Lafayette Avenue, is part of the Albany Med Health System, and Columbia Memorial Health lists it among its physician practices. The system's accepted-insurance page has separate lists of participating plans for Albany Medical Center and for Columbia Memorial Hospital, by carrier, not by individual plan. So I check the practice, the hospital its doctors use and each of your other doctors by name in the specific plan's directory.
Where is free Medicare counselling for Coxsackie residents?
Greene County's aging services, listed by the state as the Greene County Department for the Aging, are at 411 Main Street in Catskill, (518) 719-3555, and offer HIICAP, New York's free and unbiased Medicare counselling, to Greene County residents with Medicare. Appointments are required, so call ahead. The statewide HIICAP line is 1-800-701-0501. HIICAP is the second opinion I recommend to anyone who wants one.
Do you meet Coxsackie clients in person?
No. My office is in Clifton Park, and I work with Coxsackie residents by phone, WhatsApp and video. A first call takes about fifteen minutes, and a full review works the same way over video or phone.
Is there a fee?
No. There is no fee for Medicare plan help.
