Medicare agent for Lake Luzerne
The Town of Lake Luzerne sits where the Sacandaga meets the Hudson, in Warren County, with Hadley across the river in Saratoga County. That river is also a county line, so the plan list depends on which bank you live on, and the first names I check in a plan's directory are Hudson Headwaters, whose Mobile Health unit parks at Town Hall, and Glens Falls Hospital.
Where Lake Luzerne residents get care
- Hudson Headwaters Mobile Health, at the Lake Luzerne Town Hall Complex, 539 Lake Avenue (518-623-0871) — a primary care office built into a 40-foot vehicle, parked beside the Senior Center entrance on the days Hudson Headwaters posts; call to confirm before you go. It offers family medicine, screenings, gynecology, lab services, behavioral health, and nutrition and diabetes education, with walk-ins welcome; the town notes the hours can change. Hudson Headwaters' Warrensburg Health Center at 3767 Main Street adds urgent care.
- Glens Falls Hospital, 100 Park Street, Glens Falls (518-926-1000) — a member of the Albany Med Health System. The Glens Falls Hospital page places every plan sold in Warren County against the hospital's own list.
- Specialists further south. Saratoga Hospital, at 211 Church Street in Saratoga Springs, and Albany Medical Center are in the same health system as Glens Falls Hospital, but being in the same system does not mean the same insurance contracts. If you see a specialist in Saratoga Springs or Albany, I check that office separately.
Hudson Headwaters' insurance page lists Medicare and the carriers it works with, but it names carriers, not individual Medicare Advantage plans. With Medicare Advantage, the Mobile Health unit, the hospital and each specialist all have to be in the specific plan's directory for the year; Original Medicare with a Medigap policy has no network, so any provider that accepts Medicare works. The carriers each Capital Region hospital lists are on one page.
What's particular about Lake Luzerne
- The Hudson is a county line. Lake Luzerne is in Warren County, with 45 Medicare Advantage plans for 2027. Hadley, across the Hudson, is in Saratoga County, with 52 plans. Neighbors on opposite banks can see different lists, and a move from one side to the other, out of your plan's service area or to an address with new plan options, opens a Special Enrollment Period that runs for two full months after the move.
- Primary care that comes to Town Hall. If the Mobile Health unit is where you see your nurse practitioner, it is a Hudson Headwaters practice, and that is the name I look for in each plan's directory. Hudson Headwaters also has a Sliding Fee Program for people who qualify.
- Getting to appointments. The Office for the Aging lists Lake Luzerne among the towns with scheduled transportation, booked with the town's reservation clerk at least 24 hours ahead, and volunteer rides to medical appointments through the Office itself. If that's how you reach the doctor, the doctors in a plan's network have to be ones those rides can get you to.
- The pharmacy you use. The town's list of local goods and services includes a pharmacy. Part D and Medicare Advantage plans can charge different amounts at different pharmacies, so I price your prescriptions at the one you actually use, and at the plan's mail-order price where it has one.
- Medigap is priced like Albany. New York's Department of Financial Services compares Medigap premiums by region, and its Albany region covers ZIP codes starting 120–123 and 128–129, which takes in Lake Luzerne (12846). In New York you can buy or switch Medigap in any month without health questions; the current premiums for the region are on their own page.
- Retiree coverage first. State, school district and other retirees, and people still working at 65, have coverage that coordinates with Medicare in its own way. Whether an individual plan adds to it or cancels it comes before any comparison; NYSHIP retirees and Medicare while working past 65 cover the two most common cases.
Lake Luzerne Senior Center and the county Office for the Aging
The Lake Luzerne Senior Center is at the Town Hall Complex, 539 Lake Avenue, (518) 696-6451. It runs HIICAP, New York's free Medicare counselling, and helps with money-saving programs such as the Medicare Savings Program and EPIC; the HIICAP page has every county office. My primary care is on the Mobile Health unit at Town Hall. Does that matter for my Medicare plan?
Yes. Hudson Headwaters Health Network parks its Mobile Health unit at the Lake Luzerne Town Hall, 539 Lake Avenue, on the days Hudson Headwaters posts (call to confirm), with primary care, lab services, screenings and behavioral health; walk-ins are welcome and the number is 518-623-0871. Hudson Headwaters' insurance page lists Medicare and names carriers, not individual Medicare Advantage plans. Before you enroll, I check Hudson Headwaters and your provider by name in the specific plan's directory for the year.
I live in Hadley, across the river. Which plans can I get?
Saratoga County's. The Town of Hadley's own site places it in the northeastern corner of Saratoga County, separated from Lake Luzerne in Warren County by the Hudson. Medicare Advantage and Part D plans are filed by county, so neighbors on opposite banks choose from different lists, even when they use the same doctors. A move across the river can change which plans you can keep.
Do Medicare Advantage plans in Lake Luzerne include Glens Falls Hospital?
Glens Falls Hospital is a member of the Albany Med Health System and follows the system's accepted-insurance list. That list names carriers, so a carrier on it doesn't by itself mean a specific plan includes the hospital. I check the hospital and each of your doctors by name in the plan's own directory, and the Glens Falls Hospital page places every plan sold in Warren County against the hospital's list.
Where is free Medicare counselling for Lake Luzerne?
The Warren/Hamilton Counties Office for the Aging runs HIICAP, New York's free Medicare counselling program, from the Human Services Building at 1340 State Route 9 (mailing address Lake George), (518) 761-6347, office hours 8 to 4. The Lake Luzerne Senior Center at the Town Hall Complex (518-696-6451) is the town's own center. I enroll you in the plans I'm appointed with and stay your contact afterwards; HIICAP is the second opinion I recommend to anyone who wants one.
Do you meet clients in Lake Luzerne?
Yes. My office is in Clifton Park, off Northway Exit 9, and I come to you at home in Lake Luzerne, or across the river in Hadley. We can also talk by phone, WhatsApp or video. No fee for Medicare plan help.
