Medicare agent for Guilderland
Guilderland sits between the two Albany hospital systems without belonging to either: Albany Med runs the urgent care on New Karner Road, St. Peter's runs the nursing and rehab center on Mercycare Lane, and both hospitals are a short drive east. Medicare choices here come down to which of the two systems your doctors are in, and whether a plan includes both.
Where Guilderland residents get care
- Albany Med Health System — Albany Medical Center is at 43 New Scotland Avenue in Albany (518-262-3125). In town, Albany Med's EmUrgentCare Guilderland at 5 New Karner Road (518-264-9000) takes walk-ins without an appointment. It is urgent care, not a primary-care office, and I check it in the plan's directory like any other provider.
- St. Peter's Health Partners — St. Peter's Hospital at 315 South Manning Boulevard in Albany (518-525-1550), with its own primary-care and specialty practices. In Guilderland, St. Peter's runs Our Lady of Mercy Life Center at 2 Mercycare Lane (518-464-8100), a 160-bed nursing center with long-term skilled nursing care and short-term rehabilitation after a hospital stay.
- Doctors in Schenectady. If you see a doctor at Ellis in Schenectady, that is a third system with its own contracts, and I check it separately.
If your primary-care office is in one system and your specialist in the other, a plan can include one and not the other, because Albany Med and St. Peter's contract with carriers separately and the overlap changes every January. The fix is to check the directory before you enroll. The 2027 lists of Advantage carriers each system accepts are on one page, and every plan sold in Albany County is placed against each hospital's list on the Albany Medical Center and St. Peter's Hospital pages. With Original Medicare and a Medigap policy there is no network, so every one of these works as long as the provider accepts Medicare.
What's particular about Guilderland
- One town, several names. Westmere, McKownville, Guilderland Center, Fort Hunter and Altamont each have their own fire district or fire protection district, and mailing addresses vary across the town. For Medicare, the address on your card decides the county, and the whole town is in Albany County, so everyone here chooses from one list: 57 Medicare Advantage plans for 2027.
- The Schenectady County line. A move from Guilderland to Rotterdam or anywhere else in Schenectady County changes the county on your Medicare address. Plans are filed county by county, so I check the new county's list before you move. A move to an address outside your plan's service area, or to one with new plan options, opens a Special Enrollment Period.
- State and university retirees. If you worked for New York State, including at a state university, NYSHIP coordinates with Medicare in its own way. Whether a plan adds to that coverage or cancels it is the first question, before premiums or extras; the NYSHIP retirees guide covers the rules.
- Getting to appointments. The town's Senior Services office runs transportation to medical appointments, grocery stores and shopping centers for residents 60 and over (dispatch 518-631-9589). If you rely on it, it helps to keep your doctors in a system you can reach, and that is part of how I compare plans.
- Medigap is priced by region. New York's Department of Financial Services sets Medigap comparisons by region, and its Albany region covers ZIP codes starting 120–123 and 128–129, which takes in Guilderland. In New York you can buy or switch Medigap in any month without health questions; the current premiums for the region are on their own page.

Do Medicare Advantage plans in Guilderland cover both Albany Med and St. Peter's?
Some do and some don't, and the overlap changes every January. Albany Med and St. Peter's Health Partners each contract with carriers on their own, and each publishes a list that names carriers, not individual plans. If your primary care is with one system and a specialist is with the other, I check both doctors by name in the specific plan's directory before we compare anything else. If they are split, a PPO or Original Medicare with a Medigap policy is usually the safer structure.
I'm a state retiree in Guilderland with NYSHIP. Should I look at the plans I see advertised?
Not before checking with your benefits administrator. NYSHIP requires Medicare Parts A and B once you're eligible and coordinates with them, generally reimbursing the Part B premium, and joining an outside Medicare Advantage or Part D plan can disrupt that coverage. The NYSHIP retirees guide covers the rules; bring your NYSHIP paperwork to the call.
A parent may need short-term rehab at Our Lady of Mercy after a hospital stay. Does the plan matter?
Yes. Our Lady of Mercy Life Center on Mercycare Lane is part of St. Peter's Health Partners and offers short-term rehabilitation as well as long-term nursing care. With Original Medicare, a skilled nursing stay at a Medicare-certified facility follows Medicare's own rules after a qualifying hospital stay. With Medicare Advantage, the facility has to be in the plan's network and the plan often has to approve the stay first. If rehab is a realistic prospect, I check the facility in the plan's directory the same way I check doctors.
Where is free Medicare counselling for Guilderland residents?
The Albany County Department for Aging at 420 Western Avenue in Albany runs HIICAP, New York's free and unbiased Medicare counselling program; one-on-one sessions are booked at 518-447-7177. The Town of Guilderland's Senior Services office at 141 Bavarian Way in Altamont (518-280-7607) is the town's own office for residents 60 and over. HIICAP covers every plan and sells nothing; I enroll you in the plans I'm appointed with and stay your contact afterwards.
