Medicare agent for Delmar and Bethlehem

The Town of Bethlehem, Delmar, Elsmere, Slingerlands, Glenmont, Selkirk and its other hamlets, borders the City of Albany, so both of the region's big hospitals are a short drive away. The Medicare question here is less about distance than about which system your doctors belong to, and some of Delmar's primary care is with a physician group that isn't part of either.

Where Delmar and Bethlehem residents get care

  • St. Peter's Hospital, 315 South Manning Boulevard, Albany (518-525-1550) — St. Peter's Health Partners, with its own carrier list shared with Samaritan in Troy.
  • Albany Medical Center, 43 New Scotland Avenue, Albany (518-262-3125) — the region's academic medical center and the Albany Med Health System's flagship. Its practices in town include Delmar Family Medicine, 1499 New Scotland Road in Slingerlands (518-262-5057), which the system announced in August 2026 had joined it; the practice stayed in the former Slingerlands Methodist Church.
  • Community Care Physicians — Community Care Internal Medicine at the Delaware Avenue Health Park, 250 Delaware Avenue (518-439-8077), with lab, imaging, physical therapy, podiatry and audiology in the same building. The group says it is not part of a hospital or an insurance company, so it signs up with Medicare plans on its own, separately from both hospital systems.

So a Delmar household can easily have three insurance relationships to line up: a Community Care or Albany Med primary-care office, specialists at Albany Med or St. Peter's, and the hospital those specialists admit to. With a Medicare Advantage plan each of them has to be in that plan's directory for the year. Original Medicare with a Medigap policy has no network, so any provider that accepts Medicare works. Every plan sold in Albany County is placed against each hospital system's own list on the St. Peter's Hospital and Albany Medical Center pages, with all the Capital Region hospitals side by side on one page.

What's particular about Delmar and Bethlehem

  • When your doctor's practice changes hands. Delmar Family Medicine joining the Albany Med Health System is a reminder that a practice can change owners in the middle of a plan year. That can change which plans it takes, and it doesn't always line up with January. If your doctor's office has joined or left a system, I check it by name in each plan's directory for the coming year, and we read your Annual Notice of Change together.
  • Seven hamlets, one plan list. Delmar, Elsmere, Slingerlands, Glenmont, Selkirk, North Bethlehem and South Bethlehem are all in the Town of Bethlehem and all in Albany County, so they share one Medicare Advantage list: 57 plans for 2027. Across the Hudson is Rensselaer County, with its own list (57 plans); the East Greenbush page covers that side. A move out of your plan's service area, or to an address with new plan options, opens a Special Enrollment Period that runs for two full months after the move.
  • State retirees. If you retired from New York State, NYSHIP requires Medicare Parts A and B once you're eligible and coordinates with them, generally reimbursing the Part B premium. Joining an outside Medicare Advantage or Part D plan can disrupt that coverage, so that question comes before any comparison; the NYSHIP retirees guide covers it.
  • Social Security is downtown. Bethlehem is served by the Albany Social Security Office at 11A Clinton Avenue, Room 430, Albany, NY 12207 (1-866-253-9183); hours: Monday to Friday, 9 a.m. to 4 p.m.; closed weekends and federal holidays. It's in a federal building that screens visitors, so bring photo ID. Medicare Part A and B sign-up and Extra Help applications go through Social Security, and nearly all of it can be done online or by phone instead.
  • Medigap is priced as Albany. New York's Department of Financial Services compares Medigap premiums by region, and its Albany region covers ZIP codes starting 120–123 and 128–129, which takes in the whole town. In New York you can buy or switch Medigap in any month without health questions; the current premiums for the region are on their own page.

Meeting in Delmar

My office is in Clifton Park, off Northway Exit 9. I come down to meet Delmar, Slingerlands, Glenmont and Selkirk clients at home, or we talk by phone, WhatsApp or video. A first call takes about fifteen minutes: your ZIP code, your doctors and which group each one belongs to, your prescriptions, and any NYSHIP, retiree, VA or Medicaid coverage. Nothing is decided on a first call.

What a Delmar plan review covers

  1. Every doctor you see, checked against each plan's current directory: your Community Care or Albany Med primary-care office, your specialists, and the hospital they admit to.
  2. Prescriptions priced through each plan's formulary for the full year, at the pharmacy you actually use.
  3. NYSHIP, retiree, VA or Medicaid coverage and exactly how each option would interact with it, plus a screening for the Medicare Savings Program and Extra Help.
  4. A written comparison with total yearly cost, and a plain statement of whether your current coverage should be left alone.

No fee for Medicare plan help. Request a review, read the Albany page if your care is mostly in the city, or the wider Medicare page for all of Albany County.

Questions

My doctor is at Delmar Family Medicine, which joined Albany Med. Is my Medicare Advantage plan still good there?

Check before you assume either way. The Albany Med Health System announced in August 2026 that Delmar Family Medicine had joined it; the practice stayed in its building on New Scotland Road in Slingerlands. When a practice joins a health system, the plans it takes can change with the system's contracts, and the change doesn't always line up with January. I look the practice and your own doctor up by name in each plan's directory for the coming year, and read your Annual Notice of Change for anything that affects them.

My primary care is with Community Care Physicians in Delmar. Which plans include it?

Community Care Physicians says it is not part of a hospital or an insurance company, so the plans it takes are its own decision, not Albany Med's or St. Peter's. Its insurance page lists the carriers it accepts and says the list isn't complete. With Medicare Advantage I check the Delmar office and your doctor in the specific plan's directory, then check the hospital and specialists they refer you to separately. With Original Medicare and a Medigap policy, any of them works as long as they accept Medicare.

Is there help getting to doctor appointments from Delmar?

Yes. Bethlehem Senior Services and Friends of Bethlehem Seniors run curb-to-curb rides with volunteer drivers for town residents 60 and over, to medical, dental and therapy appointments and grocery shopping. Reservations are at 518-439-5770; there is no fee, though a suggested donation is posted in the vehicles. The town says demand can exceed the drivers available, so book early and have a back-up.

Where is free Medicare counselling near Delmar?

The Albany County Department for Aging at 420 Western Avenue in Albany runs HIICAP, New York's free and unbiased Medicare counselling; one-on-one sessions are booked at 518-447-7177. In town, Bethlehem Senior Services at Town Hall (518-439-4955, extension 1176) helps residents 60 and over with program eligibility and referrals. I enroll you in the plans I'm appointed with and stay your contact afterwards; HIICAP is the second opinion I recommend to anyone who wants one.

Do you meet Delmar and Bethlehem clients in person?

Yes. My office is in Clifton Park, off Northway Exit 9, and I come to you at home in Delmar, Slingerlands, Glenmont or Selkirk. A first conversation can be by phone or WhatsApp, and full reviews work well by video when that's easier.
