Medicare agent for Berne and Knox
The Town of Berne describes itself as a small community far from most medical care, with a volunteer ambulance squad that serves the Hilltowns. Berne and Knox are both Albany County towns, yet the Berne ZIP code reaches into Schoharie County, and the Albany hospitals belong to two systems that contract with carriers separately. A Medicare plan here starts with your county and your doctors.
Where Berne and Knox residents get care
The Town of Berne describes itself as a small community far from most medical care. Albany's two hospital systems contract with Medicare Advantage carriers separately:
- Albany Medical Center, 43 New Scotland Avenue, Albany (518-262-3125) — the region's academic medical center and the flagship of the Albany Med Health System.
- St. Peter's Hospital, 315 South Manning Boulevard, Albany (518-525-1550) — part of St. Peter's Health Partners, with its own primary-care and specialty practices.
In an emergency, Original Medicare and every Medicare Advantage plan cover emergency care at any hospital. The network matters for everything else: with a Medicare Advantage HMO, your primary-care doctor, your specialists and the hospital they admit to all have to be in the plan's directory for the year. Original Medicare with a Medigap policy has no network, so any provider that accepts Medicare works. Every plan sold in Albany County is placed against each hospital's carrier list on the Albany Medical Center and St. Peter's Hospital pages, and one page has every Capital Region hospital side by side. I still check each hospital and doctor by name in the plan's directory.
What's particular about Berne and Knox
- A Berne address can be in Schoharie County. The Census Bureau's ZIP-to-county file puts most of the land in Berne's 12023 ZIP area in Albany County, but part of it in Schoharie County. Plans follow the county you live in, not the post office: Albany County has 57 Medicare Advantage plans for 2027, and Schoharie County has 34 plans. East Berne's 12059 ZIP area is entirely in Albany County. Knox's own Town Hall, at 2192 Berne-Altamont Road, has an Altamont address, and Altamont's 12009 ZIP area is almost all in Albany County with a small part in Schenectady County (57 plans). A move out of your plan's service area, or to an address with new plan options, opens a Special Enrollment Period that runs for two full months after the move.
- A volunteer ambulance. The Helderberg Ambulance squad, based at 978 Cole Hill Road in East Berne and listed among the Town of Berne's departments, is looking for volunteers; the town calls it vital to the community and asks for help so it can keep serving the Hilltowns.
- Getting to Albany. For county residents 60 and over who have no other way to get to vital appointments, the Albany County Department for Aging arranges rides through CDTA ACCESS (518-437-5161, at least 48 hours ahead; dialysis and medical visits first, rides depend on availability). If that's how you get to the doctor, the plan's doctors have to be ones those rides can reach.
- Medigap is priced like Albany. New York's Department of Financial Services compares Medigap premiums by region, and its Albany region covers ZIP codes starting 120–123 and 128–129, which takes in Berne (12023), East Berne (12059) and Altamont (12009). In New York you can buy or switch Medigap in any month without health questions; the current premiums for the region are on their own page.
- Retiree and employer coverage first. State, school district and hospital retirees, and people still working at 65, have coverage that coordinates with Medicare in its own way. Whether an individual plan adds to it or cancels it comes before any comparison; NYSHIP retirees and Medicare while working past 65 cover the two most common cases.

My mailing address says Berne. Which county's plans can I get?
The county you live in, which is not always the one the post office suggests. Medicare Advantage and Part D plans are filed by county. The Census Bureau's ZIP-to-county file puts most of the land in Berne's 12023 ZIP area in Albany County but part of it in Schoharie County, which has its own plan list. East Berne's 12059 ZIP area is entirely in Albany County, and Altamont's 12009, which the Town of Knox uses for its Town Hall, is almost all in Albany County with a small part in Schenectady County. I confirm the county from your street address before we compare anything.
Which hospital do Medicare Advantage plans here use?
The hospitals are in Albany: Albany Medical Center, part of the Albany Med Health System, and St. Peter's Hospital, part of St. Peter's Health Partners. The two contract with carriers separately and each publishes a list of carriers, not plans, so a plan can include one and not the other. I check each hospital and doctor by name in the plan's directory.
I don't drive. How do I get to appointments in Albany?
Albany County's Department for Aging arranges rides through CDTA ACCESS for county residents 60 and over who have no other way to get to vital appointments, with dialysis and medical appointments first in line. Call 518-437-5161 at least 48 hours ahead; rides depend on availability. If that's how you get to the doctor, I check that a plan's doctors are ones you can reach that way.
Where is free Medicare counselling for Berne and Knox residents?
The Albany County Department for Aging at 420 Western Avenue in Albany (518-447-7198) runs HIICAP, New York's free and unbiased Medicare counselling program; one-on-one sessions are booked at 518-447-7177, and the statewide line, 1-800-701-0501, connects you to it. In Knox, the town's Senior Services Coordinator is at 518-872-7105. I enroll you in the plans I'm appointed with and stay your contact afterwards; HIICAP is the second opinion I recommend to anyone who wants one.
