Medicare agent for Cohoes

Cohoes sits where the Mohawk meets the Hudson, with Saratoga County across the Mohawk and Troy, the seat of Rensselaer County, on the east side of the Hudson. The city is in Albany County, and that decides which Medicare Advantage plans you can choose from, whichever side of either river your doctors are on.

Where Cohoes residents get care

Cohoes has primary care in the city and hospitals in Troy and Albany, run by two health systems that contract with Medicare Advantage carriers separately:

  • St. Peter's Health Partners — Cohoes Family Care at 55 Mohawk Street, Suite 105 (518-235-7282), with adult and geriatric care, and a St. Peter's Laboratory in the same building (518-525-1475, weekdays 7 to 3). Admissions go to Samaritan Hospital, 2215 Burdett Avenue in Troy (518-271-3300), or St. Peter's Hospital, 315 South Manning Boulevard in Albany (518-525-1550); both follow the St. Peter's carrier list. The system also runs Eddy Village Green at 421 Columbia Street (518-237-5630), a skilled nursing and short-term rehabilitation center.
  • Albany Med Health System — EmUrgentCare – Latham at 1019 New Loudon Road, which has a Cohoes mailing address (518-264-9000; Sunday to Friday, 9 to 8, no appointment needed). Admissions go to Albany Medical Center, 43 New Scotland Avenue in Albany (518-262-3125), which publishes its own carrier list.

With a Medicare Advantage HMO, your primary-care doctor, your specialists, the hospital they admit to and, after a hospital stay, the rehab or nursing facility all have to be in the plan's directory for the year. Original Medicare with a Medigap policy has no network, so any provider that accepts Medicare works. The pages for Samaritan, St. Peter's and Albany Med show which carriers each one lists, and one page has them side by side; I still check each hospital and doctor by name in the plan's directory.

What's particular about Cohoes

  • Two rivers, three counties. The Mohawk runs along the city's northern edge and the Hudson along the east side of Van Schaick Island. Cohoes sees the Albany County plan list: 57 Medicare Advantage plans for 2027. Across the Mohawk, Waterford is in Saratoga County (52 plans); on the east side of the Hudson, Troy is the seat of Rensselaer County (57 plans). Neighbors who use the same hospital can be choosing from different lists. A move out of your plan's service area, or to an address with new plan options, opens a Special Enrollment Period that runs for two full months after the move.
  • Getting to appointments. The county's Department for Aging arranges rides through CDTA ACCESS for Albany County residents 60 and over who have no other way to get to vital appointments, with dialysis and medical visits first in line (518-437-5161, at least 48 hours ahead; rides depend on availability). The same line handles STAR paratransit and CDTA's half-fare Navigator card. On regular buses, Route 182 runs seven days a week from downtown Troy through Cohoes and Latham to Albany, with a stop at Albany Memorial Hospital on Northern Boulevard, and the BusPlus Blue Line runs from Remsen Street in downtown Cohoes to Albany. If that's how you get around, the plan's doctors have to be somewhere those routes reach.
  • Medigap is priced like Albany. New York's Department of Financial Services compares Medigap premiums by region, and its Albany region covers ZIP codes starting 120–123 and 128–129, which takes in Cohoes (12047). In New York you can buy or switch Medigap in any month without health questions; the current premiums for the region are on their own page.
  • Retiree and employer coverage first. State, school district and hospital retirees, and people still working at 65, have coverage that coordinates with Medicare in its own way. Whether an individual plan adds to it or cancels it comes before any comparison; NYSHIP retirees and Medicare while working past 65 cover the two most common cases.

Meeting in Cohoes

My office is in Clifton Park, off Northway Exit 9, a drive down the Northway from Cohoes. I meet Cohoes clients there, at home, or by phone, WhatsApp or video. A first call takes about fifteen minutes: your ZIP code, your doctors and where they are, your prescriptions, and any retiree, VA or Medicaid coverage. Nothing is decided on a first call.

What a Cohoes plan review covers

  1. Every doctor you see, checked against each plan's current directory, including which hospital they admit to: Samaritan, St. Peter's or Albany Med.
  2. Prescriptions priced through each plan's formulary for the full year, at the pharmacy you actually use.
  3. Retiree, employer, VA or Medicaid coverage and exactly how each option would interact with it, plus a screening for the Medicare Savings Program and Extra Help.
  4. A written comparison with total yearly cost, and a plain statement of whether your current coverage should be left alone.

Questions

Common questions

Short answers to what people ask before they call.

Still have a question? Call (518) 902-8822 or book a call.

I live in Cohoes and my doctors are in Troy. Which plan list do I get?

Albany County's. Medicare Advantage and Part D plans are filed by county and follow where you live, not where your doctors are. Cohoes is in Albany County; Troy, on the east side of the Hudson, is the seat of Rensselaer County, and Waterford, across the Mohawk, is in Saratoga County. Each has its own list. What matters for your Troy doctors is whether they are in the directory of the Albany County plan you choose, and I check each one by name.

Which hospital do Medicare Advantage plans in Cohoes use?

It depends on your doctors. St. Peter's Health Partners has Cohoes Family Care and a lab at 55 Mohawk Street, and admits to Samaritan Hospital in Troy or St. Peter's Hospital in Albany. Albany Med runs EmUrgentCare on New Loudon Road, and its hospital is Albany Medical Center. The two systems publish separate lists of accepted Advantage carriers, so a plan can include one and not the other. I check each hospital and doctor by name in the plan's directory before we compare anything else.

I don't drive. How do I get to medical appointments from Cohoes?

Albany County's Department for Aging uses CDTA ACCESS to arrange rides for county residents 60 and over who have no other way to get to vital appointments, with dialysis and medical appointments among the priorities. Call 518-437-5161 at least 48 hours ahead; rides depend on availability. CDTA's Route 182 runs from Troy through Cohoes and Latham to Albany, stopping at Albany Memorial Hospital, and the BusPlus Blue Line runs from downtown Cohoes to Albany. When you rely on the bus, I check that a plan's doctors are somewhere you can reach.

Where is free Medicare counselling for Cohoes residents?

The Albany County Department for Aging at 420 Western Avenue in Albany (518-447-7198) runs HIICAP, New York's free and unbiased Medicare counselling program; one-on-one sessions are booked at 518-447-7177. I enroll you in the plans I'm appointed with and stay your contact afterwards; HIICAP is the second opinion I recommend to anyone who wants one.

Do you meet Cohoes clients in person?

Yes. My office is in Clifton Park, off Northway Exit 9, a drive down the Northway from Cohoes, and I also come to you at home. A first conversation can be by phone or WhatsApp; full reviews work well by video when that's easier.
