Medicare agent for Nassau
The Town of Nassau runs along Rensselaer County's southern boundary, with Sand Lake to the north, Stephentown to the east, Schodack to the west and the Columbia County towns of Chatham and New Lebanon to the south. It has two villages, Nassau on the Valatie Kill and East Nassau on the Kinderhook Creek. The county line matters for Medicare: plans follow the county you live in, and the Nassau and East Nassau ZIP codes reach into two.
Where Nassau residents get care
The hospitals Nassau and East Nassau residents are admitted to are in Troy, Albany and Hudson, and belong to two health systems:
- St. Peter's Health Partners — Samaritan Hospital at 2215 Burdett Avenue in Troy (518-271-3300) and St. Peter's Hospital at 315 South Manning Boulevard in Albany (518-525-1550). The two follow the same accepted-carrier list.
- Albany Med Health System — Albany Medical Center at 43 New Scotland Avenue in Albany (518-262-3125), and Columbia Memorial at 71 Prospect Avenue in Hudson (518-828-7601), which has care centers across Columbia and Greene counties. Being in the same system does not mean the same plan contracts for every practice.
With a Medicare Advantage HMO, your primary-care doctor, your specialists and the hospital they admit to all have to be in the plan's directory for the year; Original Medicare with a Medigap policy has no network, so any provider that accepts Medicare works. Every plan sold in Rensselaer County is placed against Samaritan's list on its own page, with St. Peter's and Albany Medical Center alongside, and the carriers each Capital Region hospital lists are on one page. Columbia Memorial has no page of its own here; I check it, and each of your doctors, by name in the plan's directory.
What's particular about Nassau
- Two villages, many hamlets, one town plan list. The Village of Nassau and the Village of East Nassau are both in the town, along with hamlets such as Alps, Brainard, Denault Corners, Dunham Hollow, Hoag's Corners and Millers Corners. All of the town is in Rensselaer County, so every home in it sees the Rensselaer County list: 57 Medicare Advantage plans for 2027.
- The ZIP codes cross the county line. The Census Bureau puts part of the Nassau (12123) and East Nassau (12062) ZIP areas in Columbia County, the county the town borders to the south. An address in that part sees the Columbia County list instead (48 plans for 2027), so I confirm the county from your street address first. A move out of your plan's service area, or to an address with new plan options, opens a Special Enrollment Period that runs for two full months after the move.
- Rides to appointments. The Rensselaer County Department for the Aging runs medical transportation for seniors who live in the county, Monday to Friday except holidays, booked at 518-270-2734 between 8 and 3:30, and an escort has to go with the senior into the doctor's office. If that's how you get to the doctor, a plan's in-network doctors have to be somewhere the van goes.
- Medigap is priced like Albany. New York's Department of Financial Services compares Medigap premiums by region, and its Albany region covers ZIP codes starting 120–123 and 128–129, which takes in Nassau (12123) and East Nassau (12062). In New York you can buy or switch Medigap in any month without health questions; the current premiums for the region are on their own page.
- Retiree and employer coverage first. State, school district and hospital retirees, and people still working at 65, have coverage that coordinates with Medicare in its own way. Whether an individual plan adds to it or cancels it comes before any comparison; NYSHIP retirees and Medicare while working past 65 cover the two most common cases.
Rensselaer County Department for the Aging, the Schodack Senior Center and Nassau Town Hall
The Rensselaer County Department for the Aging, part of Unified Family Services, is at 99 Troy Road, Suite 306, East Greenbush (518-270-2730). It runs HIICAP, New York's free and unbiased Medicare counselling: its counselors cover Medicare Advantage, Medigap, the Medicare Savings Program and EPIC, and employer or retiree coverage. The HIICAP page has every county office. Statewide HIICAP line: 1-800-701-0501.
The county's Schodack Senior Center at 1800 East Schodack Road in Castleton (518-477-6581) serves Nassau along with Castleton, Sand Lake and Schodack, with a meal program. Nassau Town Hall is at 29 Church Street (518-766-3559); the town's home page points low-income Nassau seniors to a program for small home repairs, through the Town Clerk or Supervisor, and its Advisory Committee on Seniors page has a note on help with Medicare prescription costs. The Village of Nassau's hall is at 40 Malden Street (518-766-3044).

Meeting in Nassau
My office is in Clifton Park, off Northway Exit 9. I meet Nassau and East Nassau clients at home, in either village or anywhere else in the town, or we talk by phone, WhatsApp or video. A first call takes about fifteen minutes: your street address and ZIP code, your doctors and where they are, your prescriptions, and any retiree, VA or Medicaid coverage. Nothing is decided on a first call.
What a Nassau plan review covers
- The county your home is actually in, then every doctor you see, checked against each plan's current directory, including which hospital they admit to: Samaritan, St. Peter's, Albany Medical Center or Columbia Memorial.
- Prescriptions priced through each plan's formulary for the full year, at the pharmacy you actually use.
- Retiree, employer, VA or Medicaid coverage and exactly how each option would interact with it, plus a screening for the Medicare Savings Program and Extra Help.
- A written comparison with total yearly cost, and a plain statement of whether your current coverage should be left alone.
No fee for Medicare plan help. Request a review, or read the wider Medicare page for all of Rensselaer County. Nearby: Schodack and Castleton, Averill Park, Sand Lake and Poestenkill, East Greenbush, North Greenbush and Rensselaer and, across the county line, Chatham.

Still have a question? Call (518) 902-8822 or book a call.
My address says Nassau or East Nassau. Which county's Medicare plans can I get?
The county you actually live in, not the post office name. The Census Bureau's ZIP-to-county file puts most of the Nassau (12123) and East Nassau (12062) ZIP areas in Rensselaer County and part of each in Columbia County. If your home is in the Town of Nassau, you choose from the Rensselaer County list; if it is across the line in Columbia County, the Columbia County list applies. I confirm the county from your street address before comparing anything.
Which hospital do Medicare Advantage plans in Nassau use?
Depending on your doctors you'll be admitted to Samaritan in Troy or St. Peter's in Albany, both St. Peter's Health Partners, to Albany Medical Center, or to Columbia Memorial in Hudson, which is part of the Albany Med Health System. Those hospitals publish lists of accepted carriers, not plans, so I check each hospital and doctor by name in the plan's directory for the year.
Is there a senior center for Nassau?
Rensselaer County's Schodack Senior Center at 1800 East Schodack Road in Castleton (518-477-6581) serves Castleton, Nassau, Sand Lake and Schodack, with a meal program. The town also has an Advisory Committee on Seniors, set up by the Town Board to plan and coordinate programs and services for Nassau's older residents. Neither is a Medicare office; for Medicare questions the county's Department for the Aging in East Greenbush is the place to call.
Where is free Medicare counselling for Nassau residents?
The Rensselaer County Department for the Aging, part of Unified Family Services at 99 Troy Road, Suite 306, in East Greenbush (518-270-2730), runs HIICAP. Its counselors cover Medicare Advantage, Medigap, the Medicare Savings Program, EPIC and employer or retiree coverage, and don't endorse any insurer. I enroll you in the plans I'm appointed with and stay your contact afterwards; HIICAP is the second opinion I recommend to anyone who wants one.
Do you meet Nassau clients in person?
Yes. My office is in Clifton Park, off Northway Exit 9, and I come to you at home in the Village of Nassau, East Nassau or anywhere else in the town. We can also talk by phone, WhatsApp or video when that's easier. No fee for Medicare plan help.
