Medicare agent for Hoosick Falls

The Village of Hoosick Falls is in the Town of Hoosick, in northeastern Rensselaer County, which the village describes as a ten-minute drive from Bennington, Vermont and fifty minutes from the State Capitol. Medicare choices here turn on two directions: east to Southwestern Vermont Medical Center, whose family practice is in the village, and west to the hospitals in Troy and Albany.

Where Hoosick Falls residents get care

  • Twin Rivers Medical, 16 Danforth Street, Hoosick Falls (518-686-5770) — family medicine for children, teens and adults, open weekdays, with evening hours to 8 on Mondays and Wednesdays. It is a practice of Southwestern Vermont Medical Center, so the office is in New York but the health system is in Vermont.
  • Southwestern Vermont Medical Center, 100 Hospital Drive, Bennington, Vermont (802-442-6361) — a member of Dartmouth Health that says it serves communities in Vermont, New York and Massachusetts, with emergency medicine, inpatient care and dialysis on its Bennington campus. Its billing page says it does not have contracts with all insurance companies.
  • St. Peter's Health Partners — Samaritan Hospital at 2215 Burdett Avenue in Troy (518-271-3300) and St. Peter's Hospital at 315 South Manning Boulevard in Albany (518-525-1550). The two follow the same accepted-carrier list.
  • Albany Medical Center, 43 New Scotland Avenue, Albany (518-262-3125) — the region's academic medical center, in the Albany Med Health System, which contracts with carriers separately from St. Peter's.

Original Medicare has no network, so with a Medigap policy every one of these, Vermont included, works as long as the provider accepts Medicare. With a Medicare Advantage HMO, out-of-network care is generally covered only for emergencies, out-of-area urgent care and temporary dialysis; a PPO usually lets you go out of network at a higher cost. Every plan sold in Rensselaer County is placed against Samaritan's list on its own page, with St. Peter's and Albany Medical Center alongside, and the carriers each Capital Region hospital lists are on one page. Southwestern Vermont Medical Center and Twin Rivers Medical have no page here; I check them, and each of your doctors, by name in the plan's directory.

What's particular about Hoosick Falls

  • A Vermont health system on Danforth Street. The family practice in the village belongs to a Bennington hospital that bills every insurer but does not have contracts with all of them. For anyone whose doctor is at Twin Rivers, whether a plan has the practice in its network comes before anything else. That is a reason to look at PPO plans and at Original Medicare with Medigap alongside HMOs.
  • One county list, with Washington County close by. The Town of Hoosick is in Rensselaer County, and so is virtually all of the 12090 ZIP area, so Hoosick Falls addresses see the Rensselaer County list: 57 Medicare Advantage plans for 2027. Nearby, the Eagle Bridge (12057) and Johnsonville (12094) ZIP areas cross into Washington County, which has 44 plans of its own. Plans follow the county of your street address, and a move to an address outside your plan's service area, or to one with new plan options, opens a Special Enrollment Period that runs for two full months after the move.
  • Rides to appointments. The Rensselaer County Department for the Aging runs medical transportation for seniors who live in the county, Monday to Friday, booked at 518-270-2734 between 8 and 3:30. Its pamphlet lists rides within the county and to a neighbouring county; riders must get in and out of the van on their own, and anyone who needs help into the building brings an escort. If you rely on it, ask which destinations it covers before counting on a ride to Bennington.
  • Medigap is priced like Albany. New York's Department of Financial Services compares Medigap premiums by region, and its Albany region covers ZIP codes starting 120–123 and 128–129, which takes in Hoosick Falls (12090). In New York you can buy or switch Medigap in any month without health questions; the current premiums for the region are on their own page.
  • Retiree and employer coverage first. State, school district and hospital retirees, and people still working at 65, have coverage that coordinates with Medicare in its own way. Whether an individual plan adds to it or cancels it comes before any comparison; NYSHIP retirees and Medicare while working past 65 cover the two most common cases.

Meeting in Hoosick Falls

My office is in Clifton Park, off Northway Exit 9. I meet Hoosick Falls and Town of Hoosick clients there, at home, or by phone, WhatsApp or video. A first call takes about fifteen minutes: your ZIP code, your doctors and which state they're in, your prescriptions, and any retiree, VA or Medicaid coverage. Nothing is decided on a first call.

What a Hoosick Falls plan review covers

  1. Every doctor you see, checked against each plan's current directory: Twin Rivers Medical and Southwestern Vermont Medical Center, and any doctors who admit to Samaritan, St. Peter's or Albany Medical Center.
  2. Prescriptions priced through each plan's formulary for the full year, at the pharmacy you actually use.
  3. Retiree, employer, VA or Medicaid coverage and exactly how each option would interact with it, plus a screening for the Medicare Savings Program and Extra Help.
  4. A written comparison with total yearly cost, and a plain statement of whether your current coverage should be left alone.

My doctor is at Twin Rivers Medical on Danforth Street. Which plans include it?

Twin Rivers Medical is a family practice of Southwestern Vermont Medical Center, a Vermont hospital that is a member of Dartmouth Health, even though the office is in Hoosick Falls. SVMC's billing page says it bills all insurance companies but does not have contracts with all of them, and it publishes no list of Medicare Advantage plans. So I check the practice, your own provider and the hospital they admit to by name in each plan's directory for the year. With Original Medicare and a Medigap policy, any provider that accepts Medicare works.

I see doctors in Bennington. Can a New York plan cover them?

Original Medicare with a Medigap policy covers any provider in the country that accepts Medicare, Vermont included. A Medicare Advantage plan sold in Rensselaer County may not include out-of-state providers in its network: an HMO generally covers out-of-network care only for emergencies, out-of-area urgent care and temporary out-of-area dialysis, and a PPO generally lets you go out of network at a higher cost. I check each Vermont doctor and the hospital by name in the plan's directory before you enroll.

Which plans can I get with a Hoosick Falls address?

Rensselaer County's. Medicare Advantage and Part D plans are filed by county, and the Census Bureau's ZIP-to-county file puts virtually all of the 12090 ZIP area in Rensselaer County. Some neighbouring ZIP codes are different: the 12057 (Eagle Bridge) and 12094 (Johnsonville) areas each cross into Washington County, which has its own plan list. Plans follow the county of your street address, not the town name on your mail.

Where is free Medicare counselling for Hoosick Falls residents?

The Rensselaer County Department for the Aging, part of Unified Family Services at 99 Troy Road, Suite 306, in East Greenbush (518-270-2730), runs HIICAP. Its counselors cover Medicare Advantage, Medigap, the Medicare Savings Program, EPIC and employer or retiree coverage, and don't endorse any insurer. In the village, the county's Hoosick Falls Senior Center (518-686-7561) is a meal site, not a Medicare office. I enroll you in the plans I'm appointed with and stay your contact afterwards; HIICAP is the second opinion I recommend to anyone who wants one.

Do you meet Hoosick Falls clients in person?

Yes. My office is in Clifton Park, off Northway Exit 9, and I come to you at home in Hoosick Falls or anywhere in the Town of Hoosick when that's easier. A first conversation can be by phone or WhatsApp, and full reviews work well by video. No fee for Medicare plan help.
