Medicare agent for Averill Park and Sand Lake

The Town of Sand Lake, in the southwestern corner of Rensselaer County, is three hamlets along Route 43: West Sand Lake, Averill Park and Sand Lake. Poestenkill, formed from Sand Lake in 1848, borders it to the north. The town describes itself as a 15-minute drive from downtown Albany, so Medicare choices here turn on the hospitals in Troy and Albany and the two health systems that run them.

Where Averill Park and Sand Lake residents get care

The hospitals in Troy and Albany belong to two health systems, and a Medicare plan has to fit the one your doctors use:

  • St. Peter's Health Partners — Samaritan Hospital at 2215 Burdett Avenue in Troy (518-271-3300) and St. Peter's Hospital at 315 South Manning Boulevard in Albany (518-525-1550). The two follow the same accepted-carrier list.
  • Albany Med Health System — Albany Medical Center at 43 New Scotland Avenue in Albany (518-262-3125), the region's academic medical center, which contracts with carriers separately from St. Peter's.

With a Medicare Advantage HMO, your primary-care doctor, your specialists and the hospital they admit to all have to be in the plan's directory for the year; Original Medicare with a Medigap policy has no network, so any provider that accepts Medicare works. Every plan sold in Rensselaer County is placed against Samaritan's list on its own page, with St. Peter's and Albany Medical Center alongside, and the carriers each Capital Region hospital lists are on one page. Those lists name carriers, not plans, so I still check each hospital and doctor by name in the plan's directory.

What's particular about Averill Park and Sand Lake

  • Three hamlets, two towns, one plan list. West Sand Lake, Averill Park and Sand Lake make up the Town of Sand Lake, whose Town Hall on Route 66 has an Averill Park address and a Sand Lake mailing address. Poestenkill, with its own hamlets of Poestenkill, East Poestenkill, Barberville and Ives Corners, is the next town north. All of it is Rensselaer County, and the Census Bureau puts all of the Averill Park (12018), Sand Lake (12153) and Poestenkill (12140) ZIP areas there, so every address sees the Rensselaer County list: 57 Medicare Advantage plans for 2027.
  • Albany is close; its plan list is not yours. Albany Medical Center and St. Peter's are in Albany County, which has its own list (57 plans for 2027). You still choose from Rensselaer County's plans; what matters is whether a plan on that list includes the Albany doctors you drive to. A move out of your plan's service area, or to an address with new plan options, opens a Special Enrollment Period that runs for two full months after the move.
  • Rides to appointments. The Rensselaer County Department for the Aging runs medical transportation for seniors who live in the county, Monday to Friday, booked at 518-270-2734 between 8 and 3:30, to appointments in the county or a neighbouring county. Riders must get in and out of the van on their own, and anyone who needs help into the building brings an escort. If that's how you get to the doctor, a plan's in-network doctors have to be somewhere the van goes.
  • Medigap is priced like Albany. New York's Department of Financial Services compares Medigap premiums by region, and its Albany region covers ZIP codes starting 120–123 and 128–129, which takes in Averill Park (12018), Sand Lake (12153) and Poestenkill (12140). In New York you can buy or switch Medigap in any month without health questions; the current premiums for the region are on their own page.
  • Retiree and employer coverage first. State, school district and hospital retirees, and people still working at 65, have coverage that coordinates with Medicare in its own way. Whether an individual plan adds to it or cancels it comes before any comparison; NYSHIP retirees and Medicare while working past 65 cover the two most common cases.

Meeting in Averill Park and Sand Lake

My office is in Clifton Park, off Northway Exit 9. I meet Averill Park, Sand Lake, West Sand Lake and Poestenkill clients there, at home, or by phone, WhatsApp or video. A first call takes about fifteen minutes: your ZIP code, your doctors and where they are, your prescriptions, and any retiree, VA or Medicaid coverage. Nothing is decided on a first call.

What an Averill Park plan review covers

  1. Every doctor you see, checked against each plan's current directory, including which hospital they admit to: Samaritan, St. Peter's or Albany Medical Center.
  2. Prescriptions priced through each plan's formulary for the full year, at the pharmacy you actually use.
  3. Retiree, employer, VA or Medicaid coverage and exactly how each option would interact with it, plus a screening for the Medicare Savings Program and Extra Help.
  4. A written comparison with total yearly cost, and a plain statement of whether your current coverage should be left alone.

No fee for Medicare plan help. Request a review, or read the wider Medicare page for all of Rensselaer County. Nearby: East Greenbush, North Greenbush and Rensselaer, Schodack and Castleton and Troy.

Questions

Common questions

Short answers to what people ask before they call.

Still have a question? Call (518) 902-8822 or book a call.

Which hospital do Medicare Advantage plans in Averill Park use?

Depending on your doctors you'll be admitted to Samaritan in Troy or St. Peter's in Albany, both St. Peter's Health Partners, or to Albany Medical Center. St. Peter's and Samaritan share one accepted-carrier list and Albany Medical Center has its own. Those lists name carriers, not plans, so I check each hospital and doctor by name in the plan's directory for the year.

My doctors are in Albany. Can I get Albany County's plans?

No. Medicare Advantage and Part D plans are filed by the county you live in, not the county your doctors are in. An Averill Park, Sand Lake, West Sand Lake or Poestenkill address sees the Rensselaer County plan list, and the question is whether a plan on that list includes your Albany doctors in its directory for the year.

Is there a senior center for Sand Lake and Poestenkill?

Rensselaer County's Schodack Senior Center at 1800 East Schodack Road in Castleton (518-477-6581) serves Castleton, Nassau, Sand Lake and Schodack, with a lunch program by reservation. In Poestenkill, the town's Senior Services page describes the Poestenkill Seniors, who meet on Tuesday afternoons at the Sullivan Jones VFW Post on Veterans Road. Neither is a Medicare office; for Medicare questions the county's Department for the Aging in East Greenbush is the place to call.

Where is free Medicare counselling for Averill Park and Poestenkill?

The Rensselaer County Department for the Aging, part of Unified Family Services at 99 Troy Road, Suite 306, in East Greenbush (518-270-2730), runs HIICAP. Its counselors cover Medicare Advantage, Medigap, the Medicare Savings Program, EPIC and employer or retiree coverage, and don't endorse any insurer. I enroll you in the plans I'm appointed with and stay your contact afterwards; HIICAP is the second opinion I recommend to anyone who wants one.

Do you meet Averill Park and Sand Lake clients in person?

Yes. My office is in Clifton Park, off Northway Exit 9, and I come to you at home in Averill Park, Sand Lake, West Sand Lake or Poestenkill when that's easier. A first conversation can be by phone or WhatsApp, and full reviews work well by video. No fee for Medicare plan help.
